Infection control measures and proper hand washing techniques. Hand washing has to be done to prevent the growth of microorganisms on the nails, hands, and forearms. When hands are visibly dirty or contaminated with proteinaceous material or are visibly soiled with blood or other body fluids, wash hands with soap and water, if hands are not visibly soiled, use an alcohol-based hand rub for routinely decontaminating hands. Wash hands before eating and after using a restroom. Wash hands for at least 15 seconds, covering all surfaces of the hands and fingers, rinse hands with water and dry thoroughly with a disposable towel, use towel to turn off the faucet, use hand lotions or creams to minimize the occurrence of irritant contact dermatitis associated with hand antisepsis or hand washing.
SN instructed patient on s/s of infected wound susch as: Thick green or yellow drainage, Foul odor, Redness or warmth around wound, Tenderness of surrounding area, and Swelling.
SN instructed patient to eat a balanced diet and drink fluids, increase protein and take vitamins to promote wound healing.
SN advised patient to take temperature once a day before bedtime, check for bleeding, pus, hardness, swelling, odor and any color change. If any of these are present, please let your nurse or doctor know as soon as possible. Patient verbalized understanding of instructions given.
Patient was instructed on list of measures to prevent falls at home: If use a cane put rubber tips on it. Keep the house well lit especially stairways, porches and outside walkways. Use night lights in areas such as hallways and bathroom.
Instructed on the importance of following/adhering to a no concentrated sweets, low fat/sodium diet, as directed by MD.
Instructed on the importance of following good body mechanics as a measure aimed to reducing the risk of injuries.
Instructed on the importance of using joint protection principles in all activities of daily living.
Instructed on the importance of avoiding excess sugar and salt.
Instructed on the importance of dressing warmly in cold weather as well as on the importance of avoiding extremes in temperatures.
