Catheter Teaching 2643

Hand washing performed. Assembled supplies and created sterile field. Foley catheter removed and discarded using a double bag technique. Peri hygiene performed. Donned sterile gloves Insertion site area prepped using 3 swabs betadine. Foley catheter 20 Fr. 5 ML balloon inserted using sterile technique. Blood tinged urine immediate return that cleared to yellow noted. Catheter attached to collection bag for gravity drainage. Pt. tolerated well.

Low fat diet Teaching 2644

Educated PT on diet for MS patients. PT was explained that overall, people with MS need a balanced, low-fat and high-fiber diet. Unprocessed or naturally processed foods are preferred to processed foods. This is similar to the Mediterranean diet, and the same healthy diet that's recommended for the general population. Also consider limiting alcohol as much as possible. Understanding was verbalized.

Fall precautions Teaching 2652

SN taught patient rise slowly and pause prior to ambulation to ensure proper blood pressure adjustment. Pump ankle 5-10x to reduce risk of blood pressure drop after rising from seating position. Make sure you feel your seat on the back of your legs prior to sitting. Use appropriate assistive device to ambulate. Take extra precaution while turning, around rugs/mats and avoid walking backwards.

Mastitis Teaching 2656

SN instructed patient that Mastitis is an inflammation of breast tissue that sometimes involves an infection. The inflammation results in breast pain, swelling, warmth and redness. You might also have fever and chills. Mastitis most commonly affects women who are breast-feeding (lactation mastitis).

Cefuroxime Teaching 2659

Cefuroxime, sold under the brand name Zinacef among others, is an antibiotic used to treat and prevent a number of bacterial infections. These include pneumonia, meningitis, otitis media, sepsis, urinary tract infections, and lyme disease. It is used by mouth or by injection into a vein or muscle.

LVAD Teaching 2667

SN instructed on check the left ventricular assist device (LVAD) each time you assess the patient’s vital signs. You will hear the continuous humming sound of the pump when auscultating the heart. Make sure the battery-charging station is plugged into the wall and at least two spare batteries are in the charge station; a green light indicates a full charge. Additional safety checks include assessing the driveline to ensure it’s securely in place and confirming there’s a backup system controller in the room.

Potential sources of infection include ventilators, central venous catheters, peripheral I.V. lines, and indwelling urinary catheters. Keep in mind that all hospital patients are at risk for methicillin-resistant Staphylococcus aureus infection and Clostridium difficile infection, as well as pressure injuries, which can become infected.

SN instructed patient on s/s of prostate cancer. While most prostate cancer does not cause any symptoms at all, the symptoms and signs of prostate cancer may include: frequent urination, weak or interrupted urine flow or the need to strain to empty the bladder, the urge to urinate frequently at night, blood in the urine, new onset of erectile dysfunction, pain or burning during urination, which is much less common and discomfort or pain when sitting, caused by an enlarged prostate.
