General information
SN reviewed patients medication with primary caregiver and instruct on medication compliance to better control the patients disease process. SN refilled patient’s medication box for daily routine implementation during visit. SN informed M.D. of patients elevated blood pressure and no new orders received, SN was advised to continue with patients current regimen.
SN completed patient cardiovascular and pulmonary assessment. Medication reviewed and administered. Educated staff on monitoring patient for mood changes, aggressive episodes, anxiety, and agitation related to medication SEs. Staff instructed to monitor environmental hazards to prevent falls and injuries. Staff was also encourages to encourage patient to rise slowly to prevent syncope and dizziness.
Instructed patient to take medication with food to minimize GI outset.
Instructed patient to flush all expired medication down the toilet.
Instructed patient to refill her/his medication before she/he are completely out of them.
Instructed patient to shake medication for at least one full minute if told to do so.

Educated patient on new medication Cyproheptadine hydrochloride (CH) is a first-generation antihistamine which is used as an appetite stimulant.Side effects of Cyproheptadine may cause side effects. Tell your doctor if any of these symptoms are severe or do not go away:dry mouth, nose, and throat, drowsiness, dizziness, nausea, chest congestion, headache, excitement (especially in children), muscle weakness.
