General information
SN used hand cleaner, donned gloves. Drainage bag from old catheter has clear yellow with sediments urine. SN donned sterile gloves, cleaned the perineum around the urinary meatus with chlorhexidine swabs. Flush Foley with 50 cc NS and immediately drained clear yellow urine. Then connected Foley to new drainage bag, then statlock placed on right thigh to secure catheter. Adult diaper put on patient. All items used for procedure disposed of in plastic bag, tied shut and put in household trash.
Patient was given teaching on good body mechanics to avoid injury such as: keep your back straight as you walk and also when lifting making sure to never bend at the waist. If sitting put a pillow or rolled towel to support your lower back.
Neuropathy assessment and teaching on management, medication, and alternative therapies to alleviate pain.
Instructed in the importance of including more fiber in diet if not contraindicate. Instructed to take fresh fruit, cooked or stewed fruits, green vegetables, root vegetables and whole grain food.
Instructed in that not one food is perfect. However, a wide variety of foods from each group assures the desirable intake of nutrients. A well-balanced diet is essential for good health.
Instructed patient to take medication with food to minimize GI outset.
Instructed patient to flush all expired medication down the toilet.
Instructed patient to take his/her pills with a full glass of water and drink all the water even if the pill went down with the first swallow.
Instructed patient to refill her/his medication before she/he are completely out of them.
Instructed patient to shake medication for at least one full minute if told to do so.
