SN instructed patient on diabetes and kidney problem, that diabetes mellitus ( DM ) is one of the leading cause of kidney failure in the United States. Approximately one-half of people who need dialysis have kidney disease from diabetes. With that, tight control of blood sugar must be done by avoiding concentrated sweets and high-carbohydrate content foods.Diabetic patients with hypertension have a special lower blood pressure target of less than 130 / 80 mmHg to reduce cardiovascular risk and delay progression of kidney disease.
SN instructed about the Lifevest wearable defibrillator . Is a treatment option for sudden cardiac arrest that offers patients advanced protection and monitoring as well as improved quality of life. The Lifevest is the first wearable defibrillator. Unlike an implantable cardioverter defibrillator ( ICD ), the Lifevest is worn outside the body rather than implanted in the chest. This device continuously monitors the patient's heart with dry, non-adhesive sensing electrodes to detect life - threatening abnormal heart rhythms. If a life - threatening rhythm is detected, the device alerts the patient prior to delivering a treatment shock, and thus allows a conscious patient to delay the treatment shock. If the patient becomes unconscious, the device releases a Blue™ gel over the therapy electrodes and delivers an electrical shock to restore normal rhythm.
SN instructed patient about the Restoril ( temazepam ) is a benzodiazepine. Temazepam affects chemicals in the brain that may be unbalanced in people with sleep problems ( insomnia ). Restoril is used to treat insomnia symptoms, such as trouble falling asleep or staying asleep. Restoril works by slowing down the central nervous system ( brain ), causing drowsiness which helps patients fall asleep.
SN instructed that gastroesophageal reflux disease ( GERD ) is a normal phenomenon, typically occurs transiently after eating. Gastroesophageal reflux disease ( GERD ) is a condition where the degree of exposure of esophageal mucosa to gastric contents is greater than normal. Most common symptom is heartburn, but patients may have other symptoms. Treatment of gastroesophageal reflux disease (GERD ) not only causes improvement in symptoms like heartburn, but early recognition & treatment of clinically significant gastroesophageal reflux disease ( GERD ) may prevent the development of further complications. Bleeding obstructions, even malignancy may complicate matters in the long run. Most peptic strictures can be effectively dilated by gastroenterologists without resorting to surgery. Perforation is a known complication of surgical procedures. Patient / caregiver verbalized understanding to the instruction regarding gastroesophageal reflux disease ( GERD ), stated the etiology, 2 s / sx of exacerbation and 2 preventive measures.
SN instructed about the use of Januvia ( Sitagliptin ) is used along with diet and exercise and sometimes with other medications to lower blood sugar levels in patients with type 2 diabetes. It works by increasing the amounts of certain natural substances that lower blood sugar when it is high. Over time, people who have diabetes and high blood sugar can develop serious or life - threatening complications, including heart disease, stroke, kidney problems, nerve damage, and eye problems. Taking medication, making lifestyle changes, and regularly checking your blood sugar may help to manage your diabetes and improve your health. This therapy may also decrease your chances of having a heart attack, stroke, or other diabetes - related complications such as kidney failure, nerve damage, eye problems, including changes or loss of vision, or gum disease.
SN explain that the dehydration is a major risk for patients with an ileostomy. The recommended daily fluid intake is 8 to 10 eight - ounce drinks. The patient should include electrolyte - enhanced drinks such as Gatorade or Powerade.
Sn instructed patient about diabetic wounds / ulcers have poor or delayed healing. Healing problems are caused by the peripheral arterial diseases and peripheral neuropathy that can occur with diabetes, wherein the small blood vessels in different parts of the body, especially in the extremities ( hands and feet ), grow narrower and reduce the blood circulation to those areas. A lack of circulation in the extremities can result in a reduced supply of oxygen and nutrients to the body tissue and nerves, which is necessary for healing. Over time, nerves in these areas may become damaged, decreasing the sensation of pain, temperature and touch, making patients vulnerable to injury.
Sn instructed patient on ways to prevent Urinary tract infection ( UTI ). If the patient is elderly be careful with cleaning, be sure that the perineal area is being cleansed properly. Women should always wipe themselves from the front to the back. If you are tending to perineal care, take steps to ensure that you always wipe your starting in front of the urethra and wiping towards the anus. Before wiping the area again, fold the rag to a clean section. The idea is that residue from the anus should never be dragged toward or against the urethra. Patients that wear adult diapers, or briefs, should be changed on a regular basis. They should be checked every two hours or so and they should never be allowed to sit in dirty briefs for prolonged periods. You should also wiped and cleansed after every brief change and bowel movement. Douches should never be used. The right drinks - what you drink can make a difference. Caffeinated drinks and alcohol can irritate the bladder and should be avoided as much as possible. Patient and caregiver verbalized understanding.
SN instructed patient on ways to prevent Urinary tract infection ( UTI ). Cranberry juice contains active compounds that are able to survive the digestive process. In the urinary tract system, these compounds actively work to fight bacteria. Its effectiveness at treating urinary tract infections has been proven, and regularly consuming cranberry juice can help prevent them. You don’t have to choose straight cranberry juice, it can be blended with apple juice or even water for taste without losing its effectiveness. Water helps to keep the system flushed out, preventing bacteria from accumulating and causing problems. Cranberry juice or cranberry juice cocktails should be offered on a regular basis and water intake should be tracked daily to ensure the patients are consuming enough water. Never Force yourself and wait - when you need to go the bathroom, your needs should be tended to immediately. Do not, under any circumstances, wait for a long period of time. Holding a full bladder for long periods of time can quickly lead to a urinary tract infection. Patient / caregiver verbalized understanding.
Sn instructed that Carbamazepine toxicity frequently presents with neurologic, cardiovascular, and anticholinergic symptoms. Patients with mild carbamazepine toxicity or those presenting shortly after the ingestion when absorption remains incomplete may demonstrate drowsiness, nystagmus, and tachycardia. More severe intoxication may manifest as lethargy, seizure, coma, hypotension, or arrhythmia. Signs and symptoms of anticholinergic toxicity are expected. Toxicity may be prolonged due to carbamazepine's delayed and erratic absorption.
