The patient was instructed in ruptured disk in the use and request of the back brace, support, or belt. The patient was advised in the need for complete bed rest to reduction inflammatory answers and to reduce stress in the spine. The patient was advised to use a urinal, bedpan, or bedside commode during the period of bed rest to evade walking.
Patient is unable to performed insulin injection by herself due to low vision and hand tremors. No caregiver able/willing to do that procedure due to ALF's policies. Nurse will continue search a c/g able and willing to administer insulin to patient.
The patient was instructed in ulcer peptic in take care of the incision line and dressing changes. The patient was instructed to take only approved antacids. The patient was reviewed to evade aspirin-containing drugs, ibuprofen, and steroids.
Make sure the skin remains clean and dry. Examine the skin daily. Inspect pressure areas gently. Make sure the bed linens remain dry and free of wrinkles. Pat the skin dry, do not rub
SN instructed patient about rheumatoid arthritis and how it is a chronic disease that causes pain, stiffness, swelling, and loss of function in the joints. It occurs when your immune system mistakenly starts attacking healthy tissue.
SN instructed patient to recognize signs and symptoms of high blood pressure, although it is frequently asymptomatic, like blurring of vision, fatigue, nose bleeds and chest pain. If symptoms continue go to ER, or call 911.
Patient instructed in Coronary Artery Disease (CAD) is when the heart vessels become narrow, which decreases the blood flow to the heart muscle. The narrowing of the heart vessels can be caused by plaque build-up from high cholesterol, fat, and calcium. The decrease in blood flow decreases the oxygenation of the heart muscle and leads to heart damage.
Patient instructed in congestive heart failure occurs when your heart is not able to pump enough blood to meet the needs of the rest of your body. If you have CHF, blood and extra fluid may back up into your lungs, making it difficult for you to breathe, swelling in your ankles and legs (called edema) may occur, you may experience excess fatigue with exercise.
Patient was instructed If you eat more food than is balanced with your physical activity and, in some cases, diabetes medicines, your blood glucose level may rise above your goal range. Carbohydrate-containing foods directly affect your blood glucose level after eating, so reviewing the amount of carbohydrate in your meals and snacks may be helpful in determining the cause of hyperglycemia.
SN instructed patient the skin around a fistula needs to be clean, dry, and grease-less for effective pouch adherence. Enterocutaneous fistulas (ECFs) can cause contents of the intestines or stomach to leak through a wound or opening in the skin. It also can cause: Dehydration, Diarrhea, and Malnutrition.
