Patient was instructed on adequate nutrition and hydration to minimize wound development. Encourage protein, calorie-dense foods and fluids (unless contraindicated), monitor intake, weight and skin turgor, assess and address impairments in dentition and swallowing.
Patient was instructed on how to prevent pressure ulcer. A proper skin care is crucial and involves inspecting skin daily and an individualized bathing schedule, using warm (not hot) water and mild soap. Avoid massage over bony prominences and use lubricants if skin is dry.
Patient was instructed on the optimization of wound environment. Adequate nutrition and hydration, remove nonviable tissue, maintain moisture balance, protect the wound and peri-wound skin, eliminate or minimize pain, cleanse, prevent and manage infection, control odor.
Patient was instructed on treating painful wounds. Persistent pain associated with non-healing wounds is caused by tissue or nerve damage and is influenced by dressing changes and chronic inflammation. Chronic wounds take long time to heal and patients can suffer from chronic wounds for many years.
SN instructed patient to eat a balanced diet and drink fluids, eat protein like red and white meat, eggs, beans and take vitamins from vegetables/fruits , to promote wound healing.
Instructed patient fresh fruits and vegetables eaten daily will also supply your body with other nutrients essential to wound healing such as vitamin A, copper and zinc. It may help to supplement your diet with extra vitamin C. Keep your wound dressed. Wounds heal faster if they are kept warm. 2- Instructed patient getting more sleep can help wounds heal faster eat your vegetables, stay active, don't smoke, keep the wound clean and dressed.
Instructed in signs and symptoms of complications and/or deterioration of wound status such as temperature above 100 grade, increase redness, drainage, edema, increase pain, increase size of wound.
Instructed in management and control such as diet as prescribed by MD, adequate hydration 1000-2000cc 24 hours if not contraindicated, importance of high protein (meat, legumes, eggs, daily), iron and vitamin supplements if indicated.
Instructed in management and control of wound through activity such as frequent rest periods, no overexertion, no lifting, bending or stooping. Passive and active exercises to increase vascular tone. Elevate affected extremity to promote venous return. Give pain medication, if prescribed, 30 minutes prior to any activity.
Patient was instructed on wound healing. Healing time depends on a variety of factors, such as wound size and location, pressure on the wound from walking or standing, swelling, circulation, blood glucose levels, wound care, and what is being applied to the wound. Healing may occur within weeks or require several months.
