Instructed caregiver how to prevent Pressure Ulcers for Bed bound patients: Protect the Skin 
 patient from injury - Avoid massaging the skin on bony parts of the patient body. Limit pressure
 on bony areas by changing positions frequently. If the patient in a bed, preferably every 2 hours.

Lorazepam Teaching 1572

Patient was instructed on Lorazepam a benzodiazepines a medication used to treat anxiety disorder. Patient was instructed to take medication exactly as indicated by physician and to avoid the consumption of alcohol. Patient was instructed on side effects such as: •drowsiness, dizziness, tiredness;blurred vision;sleep problems (insomnia);muscle weakness, lack of balance or coordination;amnesia or forgetfulness, trouble concentrating;nausea, vomiting,constipation;appetite changes; or skin rash. If side effect become persistent or worsen contact physician immediately.

Nystatin Teaching 1586

The patient was instructed on the new medication Nystatin, to be used according the indications; The use of this medication as directed (including the replacement of missed doses). This medication is not for any disorder other than that for which it is prescribed. Even if symptomatic relief occurs within the first few days of treatment, the patient should be advised not to interrupt or discontinue therapy until the prescribed course of treatment is completed. If symptoms of irritation develop, the patient should be advised to notify the physician promptly

Low Purine Diet Teaching 1613

SN instructed patient to follow a low purine diet to help minimize acute gout attacks by limiting meat, poultry and fish. Animal proteins are high in purine. Avoid or severely limit high-purine foods, such as organ meats, herring, anchovies and mackerel. Red meat (beef, pork and lamb), fatty fish and seafood (tuna, shrimp, lobster and scallops) are associated with increased risk of gout. Because all meat, poultry and fish contain purines, limit your intake to 4 to 6 ounces (113 to 170 grams) daily. SN instructed pt/cg to cut back on fat since saturated fat lowers the body's ability to eliminate uric acid. Also instructed patient Limit or avoid foods sweetened with high-fructose corn syrup. Fructose is the only carbohydrate known to increase uric acid. It is best to avoid beverages sweetened with high-fructose corn syrup, such as soft drinks or juice drinks. Juices that are 100 percent fruit juice do not seem to stimulate uric acid production as much. SN also discussed to choose complex carbohydrates and explained to patient/cg that pt will need to eat more whole grains and fruits and vegetables and fewer refined carbohydrates, such as white bread, cakes and candy. SN advised CG to ensure that pt. drinks plenty of fluids, particularly water. Fluids can help remove uric acid from your body.

Hypertension Teaching 1647

Patient was instructed on some potential complications of hypertension, such as: retinal damage , cerebrovascular accident (CVA) or stroke and kidney failure. Also the patient was instructed on some measures aimed to managing/controlling hypertension, such as: exercise regularly and have regular check-ups, achieve and/or maintain ideal weight, and decrease sodium intake to decrease retention of fluid and the workload of the heart as directed by MD. Patient understood instructions given.

Diarrhea Teaching 1656

Instructed caregiver although the formula has water in it, patient may need extra water to keep from getting dehydrated. Patient may need more water when sweating, such as during hot weather or if you have a fever. Patient may also need more water if you have diarrhea or if you are vomiting.

Infection Teaching 1673

Patient educated that there is good and bad bacteria in our body’s and the ABT will kill both which result in the yeast infection. Yeast infection is identify by area is likely an area that is moist it is red and has raised pumps. Patient instructed on possible yeast infections related to long term use of ABT, in areas that include but is not limited to between legs, under breast, and in between groin. Patient instructed to report any symptoms to MD or SN during visit.

Fistula Teaching 1752

Instructed patient about external fistulas cause discharge through the skin. They are accompanied by other symptoms, including: abdominal pain, painful bowel obstruction, fever, elevated white blood cell count. Prevent skin maceration and breakdown from corrosive effluent and wound infection. Patient comfort, accurate measurement of effluents, patient mobility protect skin from damage from effluent, containment of effluent, odor control.

General information Teaching 1771

SN completed assessment done on all body systems and noted patient with elevated blood pressure during visit. SN completed treatment during visit and noted no drainage on old tx, wound callused and new area found to left medial top of foot remains intact with no drainage noted. SN noted patient complaint of pain to bilateral lower extremities with +2 edema noted. SN educated primary caregiver on the importance of elevation of bilateral lower extremities as well as pain management for patient.

SN instructed patient / caregiver on intellectual disability which is a condition describing below-average intellectual function and a lack of skills necessary for daily living. SN instructed patient / caregiver on causes such as infections present at birth or occurring after birth, chromosomal abnormalities such as down syndrome, environmental. SN instructed patient / caregiver on symptoms such as lack of or slow development of motor skills. Language skills, and self-help skills, especially when compared to peers, lack of curiosity and difficulty understanding and following social rules.
