Patient instructed in congestive heart failure occurs when your heart is not able to pump enough blood to meet the needs of the rest of your body. If you have CHF, blood and extra fluid may back up into your lungs, making it difficult for you to breathe, swelling in your ankles and legs (called edema) may occur, you may experience excess fatigue with exercise.

Hypertension Teaching 1762

Instructed that medication Spironolactone is used to treat high blood pressure. Lowering high blood pressure helps prevent strokes, heart attacks, and kidney problems. It is also used to treat swelling (edema) caused by certain conditions (e.g., congestive heart failure) by removing excess fluid and improving symptoms such as breathing problems. Instructed that common side effects include, Drowsiness, dizziness, lightheadedness, stomach upset, diarrhea, nausea, vomiting, or headache.

General information Teaching 1771

SN completed assessment done on all body systems and noted patient with elevated blood pressure during visit. SN completed treatment during visit and noted no drainage on old tx, wound callused and new area found to left medial top of foot remains intact with no drainage noted. SN noted patient complaint of pain to bilateral lower extremities with +2 edema noted. SN educated primary caregiver on the importance of elevation of bilateral lower extremities as well as pain management for patient.

Valacyclovir Teaching 1926

SN instructed patient / caregiver that valacyclovir is an antiviral drug used in the management of herpes simplex, herpes zoster, and herpes B. Common adverse drug reactions are the same as for valacyclovir and include: nausea, vomiting, diarrhea and headache. Infrequent adverse effects may include: agitation, vertigo, confusion, dizziness, edema, arthralgia, sore throat, constipation, abdominal pain, rash, weakness and/or renal impairment. Rare adverse effects include: coma, seizures, neutropenia, leukopenia, tremor, ataxia, encephalopathy, psychotic symptoms, crystalluria, anorexia, fatigue, hepatitis, Stevens–Johnson syndrome, toxic epidermal necrolysis and/or anaphylaxis.
