Hydrocodone Teaching 2056

SN instructed patient / caregiver on Hydrocodone is an orally active narcotic analgesic ( pain reliever ) and antitussive ( cough suppressant ). SN instructed patient / caregiver that common side effects include dizziness, light - headedness, nausea, sedation, vomiting. SN instructed that side effects may be alleviated by lying down, and if they persist or worsen to notify physician. SN instructed to call physician immediately if the patient experiences slow / irregular breathing, slow / irregular heartbeat, change in the amount of urine or any allergic reactions. SN instructed patient / caregiver not to take more than 8 tabs in one day and wait 6 hours in between administration to avoid complications.

Percocet Teaching 2057

SN instructed patient / caregiver that Percocet is used to treat moderate to moderately severe pain - containing two drugs: acetaminophen and oxycodone. Explained that Acetaminophen is used to reduce both pain and fever & Oxycodone is used for its calming effect and for pain. Explained that the common side effects include dizziness, light - headedness, nausea, sedation, vomiting. SN instructed that side effects may be alleviated by lying down and if they persist or worsen to notify Physician. SN instructed to call physician immediately if the patient experiences slow / irregular breathing, slow / irregular heartbeat, change in the amount of urine or any allergic reactions. SN instructed patient to drink at least 8 glasses of water and eat foods that are high in fiber such as apples, peaches, oranges and oats if constipation is experienced.

Ciprofloxacin HCL Teaching 2058

SN instructed patient regarding medication Ciprofloxacin which is the antibiotic prescribed by patient's physician for UTI. SN instructed patient that the medication's common side effects include nausea, vomiting, diarrhea and abdominal pain. SN Instructed patient to notify physician if any of these symptoms persist or worsen. Emphasized to patient the importance of completing the full course of the antibiotic regimen as prescribed to achieve the maximum therapeutic effect of the medication.

Bystolic Teaching 2059

SN instructed patient / caregiver regarding medication Bystolic. SN explained to patient / caregiver that Bystolic is used to treat high blood pressure ( hypertension ). SN explained to patient that the side effects of this medication includes: headache, dizziness, tiredness, nausea, slow heartbeat and trouble sleeping. SN instructed patient / caregiver that if any of these effects persist or worsen, to notify physician.

Coumadin Teaching 2060

SN instructed patient / caregiver regarding medication Coumadin / Warfarin. SN explained to patient / caregiver that Coumadin / Warfarin is an anticoagulant which is prescribed to people with an increased tendency for thrombosis or as secondary prophylaxis ( prevention of further episodes ) in those individuals that have already formed a blood clot ( thrombus ). Explained that Warfarin treatment can help prevent formation of future blood clots and help reduce the risk of embolism. Informed that Warfarin will prolong bleeding time, instructed to avoid activities that increase risks of trauma. SN explained to patient / caregiver that the adverse effects of this medication includes: hemorrhage, nosebleeds and blood in urine or stool ( black tarry stools ). Instructed to notify physician if experiencing unusual bleeding while on this medication.

Cephalexin Teaching 2061

SN instructed patient / caregiver regarding medication Cephalexin. SN explained that Cephalexin is used to treat urinary tract infections, respiratory tract infections, and skin and soft tissue infections. SN explained that the side effects of this medication includes: diarrhea, dizziness, headache, indigestion, joint pain, stomach pain and tiredness. SN explained that the drug can also cause yellowing of the eyes or skin; red, blistered, swollen or peeling skin, unusual bruising or bleeding, decreased urination, severe cramps and confusion. SN instructed to notify physician if experiencing persistence or worsening of side effects.

Actonel Teaching 2062

SN instructed patient / caregiver that Actonel is used to treat and prevent bone loss ( osteoporosis ). SN instructed patient of the side effects of Actonel: nausea, diarrhea, constipation and headache. SN instructed patient / caregiver to report to physician of adverse effects: flu - like symptoms ( e.g., fatigue, fever, aches ), weakness, chest or stomach pain, pain when swallowing, leg or ankle swelling, joint pain, bone pain.

Actos Teaching 2063

SN instructed patient / caregiver that Actos is an antidiabetic med that is used to lower blood sugar. SN explained that it is important to not take medication when blood sugar is low. SN instructed to patient / caregiver to check blood sugar level before taking the medication to avoid hypoglycemia. SN instructed that side effects of the medication include: SOB, swelling or rapid weight gain, chest pain, N / V, jaundice, blurred vision, pale skin, easy bruising. SN instructed patient / caregiver to inform physician if experiencing any of these side effect.

Advair diskus Teaching 2064

SN instructed patient / caregiver that Advair Diskus is a combination of inhaled drugs that is used to treat asthma and chronic bronchitis. SN instructed patient / caregiver on common side effects including upper respiratory tract infection & headaches. SN instructed to call physician / nurse for adverse effects including nausea, vomiting, diarrhea, mouth or throat candidiasis, or musculoskeletal pain.

Albuterol Teaching 2065

SN instructed patient / caregiver on albuterol. SN explained that albuterol is a bronchodilator indicated in COPD for opening of the airways and decrease shortness of breath. SN explained that side effects include: nervousness, restlessness, tremor, chest pain, palpitations, headache, insomnia. SN instructed to notify physician immediately if shortness of breath is not relieved or is accompanied by diaphoresis, dizziness, palpitations, or chest pain.
