SN instructed patient to stop smoking, instructed that around 3 days after quitting, most people will experience moodiness and irritability, severe headaches, and cravings as the body readjusts. In as little as 1 month, a person's lung function begins to improve. As the lungs heal and lung capacity improves, former smokers may notice less coughing and shortness of breath.

Skin Care Teaching 2618

SN instructed patient and caregiver on preventing skin tears. In terms of prevention, protective arm sleeves are helpful. The use of paper or gentle release tapes is also a better alternative to nylon tape, when it comes to sensitive or aging skin. In addition, it is important to routinely moisturize dry skin with an appropriate moisturize barrier. As we age, hydrating dry skin helps to replenish missing skin and keep skin healthy and intact. Oral hydration is important as well. Patient and caregiver verbalize understanding instructions given.

Leg edema Teaching 560

Patient was instructed on symptoms of leg edema. If having leg edema feet and legs will initially appear swollen as the day progresses. After a period of time, however, the swelling will set in first thing in the morning and worsen throughout the day. Over time other symptoms will develop: High blood pressure, headaches, increased urination, palpitations, swollen hands and/or wrists, puffy eyes, weight gain.

Patient was instructed that diabetes can damage many parts of the body, including the mouth and teeth. Diabetes increases the risk of gum disease, cavities and tooth loss, dry mouth and a variety of oral infections.

Patient was instructed on balance. With age, a combination of factors decreases balance. The nerve endings in our feet don't detect impulses as readily and the balance control centers in our brain and our ears operate more slowly. Age-related changes in vision and hearing, even when subtle, also contribute to the decline in balance.

Patient was instructed on steps to prevent falls that might result in broken bones. Have the vision and hearing checked regularly, and wear slippers or shoes with a nonskid sole. Do exercises that improve balance and coordination. Limit alcohol use that clearly increases the risk of falling.

Nerves damages Teaching 604

Patient was instructed on diabetes complications such as nerves damage (diabetic neuropathy). Damage to the nerve that control the bladder, digestive tract and reproductive organ is called autonomic neuropathy and it can cause problems with nausea, vomiting, diarrhea and constipation.

Foot care Teaching 644

Patient was instructed about how to check both feet daily looking over them carefully every day, and be sure check between all of the toes. Blisters and infections can start between the toes, and with diabetic neuropathy, may not feel them until they've become irritated or infected. If physical challenges occur, ask a family member to help. Further teaching is needed.

Patient was instructed about the feet care: always wear shoes or slippers. Always wear socks with the shoes, since leather, plastics, and manmade shoe materials can irritate your skin and quickly bring on blisters. While the Diabetic patient might prefer the look of hose, nylon knee-highs, or thin socks, may find that these doesn't give to the toes or heels enough protection. Wear thicker socks to pad the feet and cushion any calluses or sore spots. Further teaching is needed.

Breathing Teaching 656

Patient was instructed on breathing methods. Breathe properly. Lie down on a rug or blanket on the floor with the legs straight and slightly apart, the toes pointed comfortably outwards, arms at the sides not touching the body, the palms up, and the eyes closed. This is called a "relaxed body" position. Take time to relax the body and breathe freely.
