SN instructed patient on new medications in the home. Instructed that Clarithromycin is used to treat a wide variety of bacterial infections. It works by stopping the growth of bacteria. Instructed that Diarrhea, nausea, vomiting, headache, and changes in taste may occur. If any of these effects persist or worsen, tell your doctor or pharmacist promptly.

Baclofen Teaching 2349

SN instructed patient/ caregiver about Baclofen.That it is a muscle relaxer and an antispastic agent. SN instructed to Get emergency medical help if you have signs of an allergic reaction to baclofen such as: hives; difficult breathing; swelling of your face, lips, tongue, or throat. Call your doctor at once if you have: weak or shallow breathing; confusion, hallucinations; or a seizure (convulsions). Common baclofen side effects may include: drowsiness, dizziness, weakness, tired feeling; headache; sleep problems (insomnia); nausea, constipation; or urinating more often than usual.

Instructed patient on isorsobide which is used to treat and prevent chest pain, side effects of isorsobide are headache, dizziness, lightheadedness, nausea or vomiting. Headache is often a sign that this medication is working; treat it with an aspirin or non-aspirin pain reliever as recommended by your doctor. If any of these effects persist or worsen, notify your doctor or pharmacist promptly.

Constipation Teaching 2421

Trulance (plecanatide) is used to treat chronic idiopathic constipation or irritable bowel syndrome with constipation. Trulance works by activating certain substances within your intestines that help increase fluid secretion and muscle movement. Trulance also causes changes in the consistency of your stools.

Pain Management Teaching 2437

SN instructed patient on importance of pain management. Take pain medication as ordered. Do not wait for pain to become severe. If your pain is not controlled and you are taking your medication as prescribed contact your doctor or nurse. Do not take your medication more often than prescribed without talking with your physician. Patient verbalized instruction.

Depression Teaching 2481

Patient instructed on new medication Abilify. Abilify is used to treat the symptoms of psychotic conditions such as schizophrenia and bipolar I disorder (manic depression). It is also used together with other medicines to treat major depressive disorder in adults. May be taken with or without food, take at the same time daily. Do not stop abruptly or change dosage without consulting prescriber. Notify prescriber if unusual or bothersome side effects occur.

Sulindac Teaching 2493

Instructed patient on new medication Sulindac. Sulindac is a nonsteroidal anti-inflammatory drug (NSAID). Sulindac works by reducing hormones that cause inflammation and pain in the body. Sulindac is used to treat pain or inflammation caused by arthritis, ankylosing spondylitis, tendinitis, bursitis, or gout. Patient verbalized understanding.

Metolazone Teaching 2506

Metolazone is used to treat fluid retention (edema) in people with congestive heart failure, or a kidney disorder such as nephrotic syndrome. Metolazone is also used to treat high blood pressure (hypertension). Possible side effects to be aware of include low potassium, headache, constipation, diarrhea, dizziness, drowsiness and blurred vision.

Amiodarone Teaching 2551

SN instructed patient and caregiver instructed on medication Amiodarone. This medication is used to stabilize the rhythm of your heart. Amiodarone is used to treat ventricular tachycardia or ventricular fibrillation. Take this medication at the same time daily as directed. May take with or without food. Avoid grapefruit, St. John's wort, & sunlight or tanning beds.

Congestion Teaching 2555

SN instructed that when you're congested, it’s because your nasal passages are swollen. This makes them feel blocked. The swelling leads to increased mucus production, which causes a runny nose. When DNSs such as Afrin shrink blood vessels, they reduce the inflammation and the associated mucus production providing temporary relief.
