Avandaryl Teaching 2079

SN instructed patient / caregiver on medication Avandaryl. Explained that it is a medication indicated for Diabetes Mellitus ( DM ) type II to decrease the blood sugar by stimulating the production of insulin and increasing receptor site sensitivity. SN explained to patient / caregiver that the side effects include dizziness, photosensitivity, low blood sugar, constipation, nausea and vomiting. SN instructed patient / caregiver to monitor blood sugar daily and to not skip or double doses. SN instructed patient / caregiver to notify physician ( MD ) if experiencing unusual weight gain, swelling of ankles, drowsiness, dyspnea ( SOB ), muscle cramps, weakness, sore throat, rash, unusual bleeding or if bruising occurs.

Ketorolac Teaching 2095

SN instructed patient / caregiver instructed on medication Ketorolac. SN explained that Ketorolac is a non - opioid anti - inflammatory drug that works to decrease pain by inhibiting prostaglandin synthesis and producing peripherally mediated analgesia. SN explained that the side effects of this medication includes drowsiness, gastrointestinal bleeding, diarrhea, dry mouth and constipation. SN instructed patient / caregiver to notify physician ( MD ) if experiencing persistence of side effects or if rash, itching, weight gain, edema, black stools, headache, or chills fever or muscle pain develops.

Metoprolol Teaching 2164

SN instructed patient / caregiver regarding Metoprolol Tartrate, which is in a group of drugs called beta - blockers. That is affect the heart and circulation ( blood flow through arteries and veins ). Metoprolol is used to treat angina ( chest pain ) and hypertension ( high blood pressure ). It is also used to treat or prevent heart attack. Call your doctor at once if you have any of these serious side effects: chest pain, pounding heartbeats or fluttering in your chest, feeling light - headed, fainting; feeling short of breath, even with mild exertion, swelling of your hands or feet, nausea, upper stomach pain, itching, loss of appetite, dark urine, clay - colored stools, jaundice ( yellowing of the skin or eyes ), easy bruising, unusual bleeding ( nose, mouth, vagina, or rectum ), purple or red pinpoint spots under your skin, wheezing, trouble breathing. Less serious side effects may include: dry mouth, constipation, heartburn, vomiting, diarrhea, headache, drowsiness, tired feeling, sleep problems ( insomnia ), or anxiety, nervousness. Do not suddenly stop taking metoprolol tartrate. Sharp chest pain, irregular heartbeat, and sometimes heart attack may occur if you suddenly stop metoprolol tartrate. The risk may be greater if you have certain types of heart disease. Tell your doctor or dentist that you take metoprolol tartrate before you receive any medical or dental care, emergency care or surgery.

SN instructed patient/CG to seek medical attention if you have signs of an allergic reaction to Augmentin such as: hives; difficult breathing; swelling of your face, lips, tongue, or throat. Call your doctor at once if you have: severe stomach pain, diarrhea that is watery or bloody; pale or yellowed skin, dark colored urine, fever, confusion or weakness; loss of appetite, upper stomach pain, jaundice (yellowing of the skin or eyes); easy bruising or bleeding; little or no urination; or severe skin reaction - fever, sore throat, swelling in your face or tongue, burning in your eyes, skin pain followed by a red or purple skin rash that spreads (especially in the face or upper body) and causes blistering and peeling.

SN instructed patient about Sulfamethoxazole. Call your doctor right away if you notice any of these side effects: Allergic reaction: Itching or hives, swelling in your face or hands, swelling or tingling in your mouth or throat, chest tightness, trouble breathing, Blistering, peeling, or red skin rash, Dark urine or pale stools, nausea, vomiting, loss of appetite, stomach pain, yellow skin or eyes, Chest pain, cough, or trouble breathing, Confusion, weakness, Muscle twitching, Severe diarrhea, stomach pain, cramps, bloating, Skin rash, purple spots on your skin, or very pale or yellow skin, Sore throat, fever, muscle pain, Uneven heartbeat, numbness or tingling in your hands, feet, or lips, Unusual bleeding, bruising, or weakness. If you notice these less serious side effects, talk with your doctor: Mild nausea, vomiting, or loss of appetite.

SN instructed patient/ CG about Miralax to relieve constipation. SN instructed that MiraLax is used as a laxative to treat occasional constipation or irregular bowel movements. Common MiraLax side effects may include: bloating, gas, upset stomach; dizziness; or increased sweating.Get emergency medical help if you have any signs of an allergic reaction to MiraLax such as: hives; difficult breathing; swelling of your face, lips, tongue, or throat. Stop taking this medicine and call your doctor at once if you have: severe or bloody diarrhea; rectal bleeding; blood in your stools; or severe and worsening stomach pain.

Instructed patient if you are taking a blood thinner, be sure you get instructions about how to take your medicine safely. Blood thinners can cause serious bleeding problems.

You have signs of severe bleeding, such as: A severe headache that is different from past headaches. Vomiting blood or what looks like coffee grounds. Passing maroon or very bloody stools.

SN instructed patient and caregiver on SIMVASTATIN. Simvastatin is a cholesterol-lowering medicine. It inhibits the production of cholesterol by the liver. High blood cholesterol is first treated with exercise, weight loss, and a diet low in cholesterol and saturated fats. When these measures fail, cholesterol-lowering medications such as Simvastatin can be added. Simvastatin is generally well-tolerated and side effects are rare. Minor side effects of Simvastatin include constipation, diarrhea, fatigue, gas, heartburn, and headache. Major side effects of Simvastatin include abdominal pain or cramps, blurred vision, dizziness, easy bruising or bleeding, itching, muscle pain or cramps, rash, and yellowing of the skin or eyes.
