The patient was advised the importance of protecting the extremity from temperature extremes and rapid changes in temperature because the tissue is delicate to temperature changes and refreezing causes tissue damage. The patient was recommended defensive procedures to avoid future incidents or reinjury of the frostbitten part, protective, multilayered, warm, nonconstrictive clothing avoiding of cold temperatures, exhaustion, and malnutrition. The patient was advised that there may be long-term remaining effects, increased sensitivity to cold, burning and itchy, and increasing sweating. The patient was explained that lower extremities are affected, to avoid weight bearing and offer instruction in the use of ambulatory aids. The patient was taught in the importance of elevating the affected extremity. The patient was advised in range-of-motion exercises to prevent contractures.

Rheumatic Fever Teaching 1512

The patient was instructed in rheumatic fever in the need for prophylactic antibiotic treatment before aggressive procedures that incline to bacteremia. The patient was advised in the importance of completing antibiotic therapy to prevent reappearance. The patient was advised in the hypersensitivity response to penicillin after long term use.

Hyperglycemia Teaching 1731

Instructed patient during periods of stress, the body releases so-called stress hormones, which cause a rise in blood glucose level. In the short term, this gives the body the extra energy it needs to cope with the stress. But if a person doesn’t have adequate insulin circulating in his bloodstream to enable his cells to use the extra energy, the result will be hyperglycemia. And if stress becomes chronic, hyperglycemia can also become chronic.

SN instructed patient / caregiver on symptoms of abnormal glucose such as increased thirst, frequent urination, fatigue, blurred vision. SN instructed patient / caregiver on risk factors such as weight, waist size, inactivity, age, family history. SN instructed patient / caregiver on healthy lifestyle choices that can help prevent diabetes such as eating healthy foods, getting more physical activity, losing excess pounds.

Alzheimer's disease Teaching 2021

SN instructed on Alzheimer's. Patient shouts at moments; Assessed patient’s ability for thought processing. Observed patient for cognitive functioning, memory changes, disorientation, difficulty with communication, or changes in thinking patterns. Assessed patient’s ability to cope with events, interests in surroundings and activity, motivation, and changes in memory pattern. Instructed caregiver to orient patient to environment as needed, if patient’s short term memory is intact. Using of calendars, radio, newspapers, television and so forth, are also appropriate. Assessed patient for sensory deprivation, concurrent use of CNS drugs, poor nutrition, dehydration, infection, or other concomitant disease processes. Maintain a regular daily schedule routine to prevent problems that may result from thirst, hunger, lack of sleep, or inadequate exercise. Provide positive reinforcement and feedback for positive behaviors. SN instructed family in methods to use with communication with patient: listen carefully, listen to stories even if they’ve heard them many times previously, and to avoid asking questions that the patient may not be able to answer. Divert attention to a client when agitated or dangerous behaviors like getting out of bed by climbing the fence bed. Eliminate or minimize sources of hazards in the environment. Caregiver verbalized fair understanding.
