SN instructed patient on liver failure. The early symptoms of liver failure are similar to symptoms of many other conditions. Because of this, liver failure may initially difficult to diagnose. Some of the most common initial symptoms of liver failure are: Nausea, Loss of appetite, fatigue, diarrhea. As liver failure progresses, the symptoms become more serious. The most common symptoms of advanced liver failure include: Skin
itchiness, jaundice, bleeding easily, bruising easily, swollen abdomen, mental disorientation or confusion, sweet or foul smelling breath, sleepiness, coma. Educated patient that the liver filters the Ammonia out the blood and that liver failure causes Ammonia levels to increase, which cause change of mental status, confusion, and sleepiness or fatigue.
Skilled Nursing visited PT, PT alert and oriented x 3 PERRL. SN assessed PT head to toe , Vital signs taken and recorded using universal precaution standard . Lungs Bilateral even , unlabored respiration, abdomen soft non distended, skin
warm and dry to touch , no distress and complications noted at this time. Osteoarthritis can occur in any joint. It occurs most often in the hands, knees, hips, and spine. Warning signs are: Stiffness in a joint after getting out of bed or sitting for a long time. Swelling or tenderness in one or more joints. A crunching feeling or the sound of bone rubbing on bone. Taught that eating well-balanced meals and avoiding.Taught that avoiding excess sugar intake and avoiding excess salt intake are measures aimed to preventing/controlling Osteoarthritis.Taught that using assistive and/or protective devices, as needed, is a measure aimed to preventing/controlling Osteoarthritis.
Instructed patient it’s important to routinely clean catheters to prevent infections. Wash your hands well before and after you handle your catheter. Clean the skin
around the catheter twice a day using soap and water.
Instructed patient when to call the doctor the stoma is swollen and is more than a 1/2 inch (1 cm) larger than normal.
The stoma is pulling in, below the skin
level. The stoma is bleeding more than normal.
SN to instruct patient change the bandage around the tube, the bolsters, skin
barriers, and tube attachments at least every 7 days. If your bandages, barriers, or devices get dirty or wet, change them right away, and as often as needed.
Patient instructed Cellulitis can appear on almost any part of the body. It usually shows up on damaged skin
such as inflamed wounds, dirty cuts, and areas with poor circulation. It needs to be treated by a doctor. Common symptoms include: Redness, Red streaking, Swelling, Warmth, Pain or tenderness, & Leaking of yellow, clear fluid or pus.
SN also instructed pt on the disease process of hypothyroidism. The thyroid gland controls a lot of our body functions, when it is unable to produce the amount of thyroid hormone your body needs, you will need to take this hormone orally. Take this medication everyday as ordered by your MD. Report to SN or MD if you have sensitivity to cold temperature, dry skin
, constipation, forgetfulness, chronic fatigue, decreased heart rate, depression, hair loss, weight gain, muscle stiffness and cramping, as these can be signs your medication is not the correct dose.

irritation or deep sores (ulcers) a lot of bleeding from the stoma opening or a moderate amount in the pouch that you notice several times when emptying it. Eating beets will cause some red discoloration.

Instructed patient about how help decrease edema and keep it from coming back -Protection. Keep the affected area clean, moisturized and free from injury. Dry, cracked skin
is more prone to scrapes, cuts and infection. Always wear protection on your feet if that's where the swelling typically occurs.
