Pain Teaching 1934

SN instructed patient to take pain medication before pain becomes severe to achieve better pain control. Always refill your meds before you run out of them. SN instructed patient on nonpharmacologic pain relief measures, including relaxation techniques, positioning ,etc. SN instructed to report to physician if experiencing pain level not acceptable , pain level greater than 6/10, pain medications not effective, unable to tolerate pain medications, and pain affecting ability to perform normal activities

Skin Care Teaching 1966

SN instructed patient on importance of protecting skin by: keeping your skin moist with lotions or ointments to prevent cracking, wearing shoes that fit well and provide enough room for your feet, learning how to trim your nails to avoid harming the skin around them, wearing appropriate protective equipment when participating in work or sports.

Hypertension Teaching 1970

SN instructed patient about some measures aimed to managing & controlling hypertension, such as: eating low sodium diet , increase more fruits to increase your potassium, walk daily for 30 minutes, and have regular check-ups, as directed by Physician.

Carbohydrates Teaching 1971

SN instructed patient about pay attention to the carbohydrates because foods that contain carbohydrates raise blood glucose. Examples of foods that contain carbohydrates are pasta, bread, rice, desserts, juice, soda, and fruit.

Blood pressure Teaching 1978

SN instructed patient about some things that he can do to help control your blood pressure , including: Eat a heart-healthy diet, including potassium ( bananas ) and fiber ( cereals,beans ) , drink plenty of water, exercise regularly : walking for a few minutes every day , limit the amount of sodium ( salt ) you eat ( avoid canned foods ), reduce stress by trying to avoid things that cause you stress.

Phantom pain Teaching 2015

SN instructed patient that phantom pain is very real but not everyone who has an amputation has phantom pain. The pain may feel like cramping, burning, stabbing or shooting pain. Some things that can make the pain feel worse are being too tired, too much pressure on the amputated arm or leg, changes in the weather, stress and infection.

Hemodialysis Teaching 2041

SN instructed that check the access for signs of infection or problems with blood flow before each hemodialysis treatment, even if the patient is inserting the needles. Keeping the access clean at all times. Using the access site only for dialysis. Being careful not to bump or cut the access. Checking the thrill in the access every day. The thrill is the rhythmic vibration a person can feel over the vascular access. Watching for and reporting signs of infection, including redness, tenderness, or pus. Not letting anyone put a blood pressure cuff on the access arm. Not wearing jewelry or tight clothes over the access site. Not sleeping with the access arm under the head or body. Not lifting heavy objects or putting pressure on the access arm.

Colostomy care Teaching 2049

SN instructed patient on Colostomy Care. You contact your healthcare provider if: You have a fever, you have a foul odor coming from your colostomy bag or stomach that lasts longer than a week, your skin around the stomach becomes red and irritated, you have nausea, vomiting, pain, cramping, or bloating, you do not have regular bowel movements through your stomach, the size of your stomach changes, you have questions or concerns about your condition.

Congestive Heart Failure Teaching 2066

SN instructed patient about that swelling of ankle or hands or increased dyspnea ( SOB ) or moist coughing is a sign of congestive heart failure ( CHF ) and contact with physician is needed reporting symptoms. Patient was able to be taught back.

Fever Teaching 2076

SN instructed patient that to drink plenty of fluids ( fever, which may be related to the flu, can cause dehydration ). It is important to maintain hydration. Take acetaminophen ( but do not take aspirin unless your doctor tells ). SN instructed patient to get a flu shot each year and decrease the exposure to the flu.
