SN instructed patient about rheumatoid arthritis and how it is a chronic disease that causes pain
, stiffness, swelling, and loss of function in the joints. It occurs when your immune system mistakenly starts attacking healthy tissue.
SN instructed patient to recognize signs and symptoms of high blood pressure, although it is frequently asymptomatic, like blurring of vision, fatigue, nose bleeds and chest pain
. If symptoms continue go to ER, or call 911.
Patient also instructed on Clostridium difficile colitis (C. diff) and the increase risk due to the multiple ABT. Patient instructed that the most common symptoms of Clostridium difficile colitis (C. diff) infection include: watery diarrhea, fever, loss of appetite, nausea, belly pain
and tenderness. Instructed to clean their hands with soap and water or an alcohol-based hand rub often and every after going to the restroom.
SN instructed patient and caregiver about what angina pectoris is. Angina pectoris is chest pain
that is caused by insufficient oxygen to meet demands of the heart. Lack of oxygen occurs when insufficient blood flows through the coronary arteries. Stable angina usually has a precipitating cause while unstable angina can occur at rest.
Instructed patient about external fistulas cause discharge through the skin. They are accompanied by other symptoms, including: abdominal pain
, pain
ful bowel obstruction, fever, elevated white blood cell count. Prevent skin maceration and breakdown from corrosive effluent and wound infection. Patient comfort, accurate measurement of effluents, patient mobility protect skin from damage from effluent, containment of effluent, odor control.
SN instructed patient to always assess wound dry sterile dressing when removed for any symptoms / signs of infection, such as increase drainage amount, any odor, drainage color, etc . Check your temperature once or twice a day. Report any fever or increase pain

SN instructed patient and caregiver about Lymphedema disease, also known as lymphatic obstruction. Is a condition of localized fluid retention and tissue swelling caused by a compromised lymphatic system, which normally returns interstitial fluid to the thoracic duct and then the bloodstream. The condition can be inherited or can be caused by a birth defect, though it is frequently caused by cancer treatments, and by parasitic infections. Though incurable and progressive, a number of treatments can ameliorate symptoms. Tissues with lymphedema are at high risk of infection. Symptoms may include a feeling of heaviness or fullness, edema and ( occasionally ) aching pain
in the affected area. In advanced lymphedema, there may be the presence of skin changes such as discoloration, verrucous ( wart-like ) hyperplasia, hyperkeratosis, and papillomatosis; and eventually deformity ( elephantiasis ). Patient verbalized understanding the teaching.
SN instructed patient on Colostomy Care. You contact your healthcare provider if: You have a fever, you have a foul odor coming from your colostomy bag or stomach that lasts longer than a week, your skin around the stomach becomes red and irritated, you have nausea, vomiting, pain
, cramping, or bloating, you do not have regular bowel movements through your stomach, the size of your stomach changes, you have questions or concerns about your condition.
SN instructed patient / caregiver that Atrial fibrillation is an irregular and often rapid heart rate that can increase your risk of stroke, heart failure and other heart-related complications. It may lead to complications. Atrial fibrillation can lead to blood clots forming in the heart that may circulate to other organs and lead to blocked blood flow ( ischemia ). Treatments for this may include medications and other interventions to try to alter the heart's electrical system. Signs and symptoms ( S / S ) such as: Palpitations, which are sensations of a racing, uncomfortable, irregular heartbeat or a flip - flopping in your chest, weakness, fatigue, lightheadedness, dizziness, chest pain
