Instructed patient on medication Amiodarone. This medication is used to stabilize the rhythm of your heart. Amiodarone is used to treat ventricular tachycardia or ventricular fibrillation. Take this medication at the same time daily as directed. May take with or without food. Avoid grapefruit, St. John's wort, & sunlight or tanning beds. common side effects include: nausea, vomiting, loss of appetite; or constipation. Call your doctor or get medical help at once if you have: chest pain, fast or pounding heartbeats, trouble breathing, vision problems, upper stomach pain, vomiting, dark urine, jaundice (yellowing of the skin or eyes), or if you cough up blood. Tell your doctor if you have signs of a thyroid problem, such as weight changes, extreme tiredness, dry skin, thinning hair, feeling too hot or too cold, irregular menstrual periods, or swelling in your neck (goiter).
Patient requires skilled intervention of physical therapy for exercises to increase strength and endurance, implementation of safety measures and evaluation for assistive devices due to impaired function or declining functional ability as demonstrated by increased weakness. Patient requires skilled intervention of physical therapy to teach and train patient/caregiver on exercises to increase strength and endurance, implementation of safety measures and use of assistive devices due to impaired function or declining functional ability to perform ADLs/IADLs. Patient with moderate pain interfering with functional status, thereby impacting activities of daily living.
SN did head to toe assessment and vital signs monitoring done. SN instructed patient on medication Losartan, it is used to treat high blood pressure (hypertension). Lowering high blood pressure helps prevent strokes, heart attacks, and kidney problems. It works by relaxing blood vessels so that blood can flow more easily. Dizziness, lightheadedness, or upset stomach may occur. If any of these effects persist or worsen, tell your doctor or pharmacist promptly. To reduce the risk of dizziness and lightheadedness, get up slowly when rising from a sitting or lying position.
SN did head to toe assessment and vital signs monitoring done. Received patient sitting on the chair with elevated blood pressure. SN instructed patient on the disease process, and management of CHF. Heart failure occurs when the heart muscle doesn't pump blood as well as it should. When this happens, blood often backs up and fluid can build up in the lungs, causing shortness of breath. Certain heart conditions gradually leave the heart too weak or stiff to fill and pump blood properly. SN instructed patient to have a healthy lifestyle. Try to lose weight, exercise, use less salt and manage stress. SN will continue to assess and instruct the patient.
Instructed patient in position and reposition every hour to prevent skin breakdown.
SN instructed patient about poor technique and incorrect landmarking of the injection site can lead to site reactions , suboptimal medication absorption and adverse events.
