General information
Instructed patient about foods low in cholesterol, such as, fish, low fat dairy products, fruits and vegetables, grains, herbs and spices, lean meats, lean poultry, egg substitutes, polyunsaturated oils and others.
SN completed assessment done on all body systems and noted patient with elevated blood pressure during visit. SN completed treatment during visit and noted no drainage on old tx, wound callused and new area found to left medial top of foot remains intact with no drainage noted. SN noted patient complaint of pain to bilateral lower extremities with +2 edema noted. SN educated primary caregiver on the importance of elevation of bilateral lower extremities as well as pain management for patient.
Skilled nurse developed patient plan of care with patient/caregiver involvement to be countersigned by physician. SN to perform complete physical assessment each visit with emphasis on disease process. SN to assess other co-morbidities including list diseases and other conditions that present themselves during this episode of care. SN to recognize and intervene to minimize complications; notify physician immediately of any potential problems that impede completion of patient recovery and desired goals.
Monitor circulation r/t compression dressing. Report if dressing is tight and constricting esp. in back of leg. Check color of feet and report if bluish or purple in color. Report any c/o numbness or tingling. Dressing should be removed immediately if any s/s occur. and notify doctor or nurse.
Instructed patient how prevent a decrease in physical activity. Prevent a decrease in mental activity. Encourage daily verbal communication. Be attentive and listen and accept their feelings. Request a consultation with physicians and rehabilitation staff, rehabilitation making regular visits to elderly evacuees.
SN instructed caregiver that the medical home can schedule health maintenance visits frequently enough to be proactive about new issues, ensure that the family has access to reliable information, community services, and resources and coordinate care and interpret information or advice from specialists
SN instructed patient that the following are the common symptoms of spinal stenosis: Tingling or numbness in legs or arms, Stiffness and severe back pain, difficulty in walking and standing for long time, might experience balancing problems and resting may provide temporary relief but the symptoms reoccur.
Patient was instructed on appropriate nutrition. Choose non fat dairy product such as skim milk, non fat yogurt and non fat cheese. Choose water and calorie free
Patient was instructed on appropriate nutrition. Cut back on high calorie snack foods and desserts like chips, cookies, cakes, and full fat ice cream. Eating too much of even healthful foods can lead to weight gain. Watch the portion sizes.
Instructed patient to avoid candies, chocolates, cookies, drinks and others foods rich in sugar. Patient was instructed on the importance of a good nutrition to control hyperglycemia: Avoid skipping meals. This can make people extra hungry, moody and unable to focus. Pay attention to the portion. Eat healthy.
