Instructed patient to follow a healthy eating plan that includes his/her favorites foods, but with the proper number of calories and sugars that they should eat daily.
Patient was instructed on the importance to have a good blood sugar control to avoid future diabetes complications: Monitoring: keep track of the blood sugar every day, Meals: Plan healthy and enjoyable meals to help keep the blood sugar near goal, Moves (exercise): moving the body help lower the blood sugar by burning it for energy, Medicine: when Meals and Move are not enough.
Patient was instructed on Hyperglycemia. Diabetes can be reduced making changes in diet and increasing physical activity. It is recommended to maintain an healthy weight having a modest fat intake and eating sufficient fiber.
Patient was instructed on how to avoid hypoglycemia. Start off every day with a healthy breakfast containing fruits, vegetables and high fiber foods such as oatmeal. Eat frequent small meals (rich in complex carbs, protein and fiber) throughout the day.
Patient was instructed on how stress can affect blood sugar. Stress can affect blood sugar in two ways: when under heavy stress, it is easy to abandon the usual routine, eating fewer healthy foods, in that sense, stress indirectly causes the blood sugar to rise.
The patient was instructed in bone marrow suppression that will be more susceptible to infection, bleeding, and anemia. The patient was encouraged in the prevention of the infection by eating healthy meals, keep mouth, teeth and gums clean, avoid people who are sick. The patient was encouraged in the prevention of the bleeding avoiding physical activities that could cause injuries. The patient was encouraged in the prevention in the anemia by eating a high-protein diet, and multivitamin supplement with minerals.
Instructed patient you can do many things to help control your blood pressure at home, including: Eat a heart-healthy diet, including potassium and fiber, and drink plenty of water, exercise regularly at least 30 minutes of aerobic exercise a day, limit the amount of sodium (salt) you eat and aim for less than 1,500 mg per day. Reduce stress by trying to avoid things that cause you stress.
SN instructed patient about rheumatoid arthritis and how it is a chronic disease that causes pain, stiffness, swelling, and loss of function in the joints. It occurs when your immune system mistakenly starts attacking healthy tissue.
SN instructed that if you have diabetes, your body cannot make or properly use insulin. This leads to high blood glucose, or blood sugar, levels. Healthy eating helps keep your blood sugar in your target range. It is a critical part of managing your diabetes, because controlling your blood sugar can prevent the complications of diabetes.
SN instructed patient / caregiver on factors that increase risk of HTN, such as: age, race, family history, stress, high sodium intake, high cholesterol intake, obesity and sedentary lifestyle. SN instructed patient / caregiver on measures to control/manage HTN, such as: exercise as tolerated, achieve/maintain healthy/ideal weight, decrease sodium intake to decrease fluid retention and the workload of the heart. SN Instructed importance of eating foods rich in calcium, protein and fiber.
