Patient recently discharge from acute hospital s/p intracranial hemorrhage and SN educated patient on s/s such as Weakne
ss., Tingling, weakne
ss, or paralysis (loss of movement) on one side of the body. Difficulty speaking, disorientation and confusion, loss of or change in vision, difficulty swallowing, change in level of consciousness (alertness) and advised patient and caregiver to contact medical professional if any noted.
Instructed to recognize S/S of Rheumatoid arthritis such as loss of appetite, fatigue, weakne
ss, pain and tenderness of joint, weight loss, swelling, etc.
Instructed in how to recognize S/S of hypotension such as dizziness, blurred vision, fatigue, weakne
ss, confusion, etc.
Instructed in how to recognize signs and symptoms of high blood sugar such as frequent urination, excessive thirst, headache, weakne
ss, fatigue, dizziness, nausea, vomiting, abdominal cramps, etc.
Instructed in possible effects of a stroke such as paralysis or weakne
ss on one side of the body, difficulty with speech and language, urine and fecal incontinence, visual deficits, emotional lability.
Instructed in complication requiring medical intervention associated with hypertension such as SOB, blurred vision, headache and extreme weakne
ss.
Instructed in the possible side effects of NTG, including headache, dizziness with position changes, weakne
ss, flushing, palpitations, fainting, nausea and vomiting. Notify MD should any side effects occur.
Instructed in other S/S of cardiac complications requiring medical intervention, such as, increased SOB, palpitations, uneven heart beat, faintness, and weakne
ss.
Instructed patient on how to recognize S/S of hypotension, such as, dizziness, blurred vision, fatigue, weakne
ss, confusion, etc.
Instructed patient about symptoms of stroke: weakne
ss or numbness on one side of the face or body, including a leg or an arm. Sudden trouble seeing with one or both eyes. Double vision. Trouble talking, such as slurred speech. Problems understanding or using words. Severe headache. Dizziness or loss of balance. Any of these symptoms can come and go without warning.
