Instructed in the importance of exercise. This avoids phlebitis, decubitus ulcer, pneumonia, fractures, depression, urinary complications, muscle weakness and atrophy and constipation.

The patient was instructed in pyelonephritis in the need of stopping repeated urinary tract infection. The patient was advised to keep the perineal area clean and dry. The patient was taught the method of finding midstream urine samples.

Intertrigo commonly is seasonal, associated with heat and humidity or strenuous activity in which chafing occurs. In addition to obesity and diabetes, hyperhidrosis may be a risk factor for intertrigo. Additional factors that predispose individuals to perineal intertrigo include urinary or fecal incontinence, vaginal discharge, or a draining wound.
SN instructed patient / caregiver about Diphenhydramine( Benadryl). It is a first-generation antihistamine used to treat a number of conditions including allergic symptoms and itchiness, the common cold, insomnia, motion sickness, and extrapyramidal symptoms. It is a potent anticholinergic agent. This activity is responsible for multiples side effects such as: dry mouth and throat, increased heart rate, pupil dilation, urinary retention, constipation, and, at high doses, hallucinations or delirium. Other side effects include motor impairment, flushed skin, blurred vision at near point owing to lack of accommodation, abnormal sensitivity to bright light (photophobia), sedation, difficulty concentrating, short-term memory loss, visual disturbances, irregular breathing, dizziness, irritability, itchy skin, confusion, increased body temperature, temporary erectile dysfunction, and excitability, and although it can be used to treat nausea, higher doses may cause vomiting. Report any Side effect / Adverse reaction to Physician / Nurse as needed.
SN instructed patient that Vesicare (solifenacin) reduces muscle spasms of the bladder and urinary tract. It is used to treat symptoms of overactive bladder, such as frequent or urgent urination, and incontinence ( urine leakage) . The most common side effect of solifenacin are dry mouth, blurred vision, and constipation. As all anticholinergics ,it may rarely cause hyperthermia due to decreased perspiration.
SN instructed thats cystitis is inflammation of the bladder, usually caused by a bladder infection. It's a common type of urinary tract infection ( UTI ), particularly in women, and is usually more of a nuisance than a cause for serious concern. Mild cases will often get better by themselves within a few days. However, some people experience episodes of cystitis frequently and may need regular or long - term treatment. There's also a chance that cystitis could lead to a more serious kidney infection in some cases, so it's important to seek medical advice if your symptoms don't improve.
SN instructed that the most commonly experienced symptom across individuals with UTIs is a burning sensation during urination. The passage through which urine passes to be expelled from the body becomes tender due to the infection; when urine goes through it, the person experiences a painful sensation that mostly doesn’t last post urination. Since in a lot of cases UTIs are not present with signs, if a burning pain is felt during urination, the infection may be strong, and thus seeking treatment at the earliest is recommended.
