respiratory failure
SN instructed patient / caregiver that bumetanide ( trade names Bumex or Burinex ) is a loop diuretic to treat heart failure
. It is often used in people in whom high doses of furosemide are ineffective. Instructed that is used to reduce extra fluid in the body ( edema ) caused by conditions such as heart failure
, liver disease, and kidney disease. This can lessen symptoms such as shortness of breath and swelling in your arms, legs, and abdomen. Bumetanide is a “ water pill ” ( diuretic ) that causes you to make more urine. This helps your body get rid of extra water and salt. Dizziness may occur as your body adjusts to the medication. Tell your doctor or pharmacist promptly, If this effect persists or worsens. To reduce the risk of dizziness and lightheadedness, get up slowly when rising from a sitting or lying position. Many people using this medication do not have serious side effects. It may cause a serious loss of body water ( dehydration ) and salt / minerals. Tell your doctor right away if you have any of these unlikely but serious side effects such as: muscle cramps, weakness, unusual tiredness, confusion, severe dizziness, fainting, drowsiness, unusual dry mouth / thirst, nausea, vomiting, fast / irregular heartbeat, unusual decrease in the amount of urine.
SN explained that the symptoms / signs of congestive heart failure
( CHF ) may include: Shortness of breath ( also called dyspnea ) most commonly during activity, at rest, or while sleeping, which may come on suddenly and wake you up. You often have difficulty breathing while lying flat and may need to prop up the upper body and head on two pillows. You often complain of waking up tired or feeling anxious and restless. Blood backs up in the pulmonary veins ( the vessels that return blood from the lungs to the heart ) because the heart can't keep up with the supply. This causes fluid to leak into the lungs. Persistent coughing or wheezing, coughing that produces white or pink blood - tinged mucus. Fluid builds up in the lungs. Buildup of excess fluid in body tissues ( edema ) swelling in the feet, ankles, legs or abdomen or weight gain. You may find that your shoes feel tight.
SN explain about congestive heart failure
( CHF ). As blood flow out of the heart slows, blood returning to the heart through the veins backs up, causing fluid to build up in the tissues. The kidneys are less able to dispose of sodium and water, also causing fluid retention in the tissues. Fatigue, a tired feeling all the time and difficulty with everyday activities, such as shopping, climbing stairs, carrying groceries or walking. The heart can't pump enough blood to meet the needs of body tissues. The body diverts blood away from less vital organs, particularly muscles in the limbs, and sends it to the heart and brain. Lack of appetite, nausea, a feeling of being full or sick to your stomach. The digestive system receives less blood, causing problems with digestion. Confusion, impaired thinking, memory loss and feelings of disorientation. A caregiver or relative may notice this first. Changing levels of certain substances in the blood, such as sodium, can cause confusion. If you have 2 or more of these symptoms you may need to seek medical attention.
Patient and CG instructed on Pravastatin. Pravastatin is used to lower cholesterol and triglycerides (types of fat) in the blood. Pravastatin is also used to lower the risk of stroke, heart attack, and other heart complications in people with or without coronary heart disease or other risk factors. Pravastatin can cause a condition that results in the breakdown of skeletal muscle tissue, leading to kidney failure
. This condition may be more likely to occur in older adults and in people who have kidney disease or poorly controlled hypothyroidism (underactive thyroid). Get emergency medical help if you have signs of an allergic reaction to pravastatin: hives; difficulty breathing; swelling of your face, lips, tongue, or throat. In rare cases, pravastatin can cause a condition that results in the breakdown of skeletal muscle tissue, leading to kidney failure
. Stop using and call your doctor at once if you have:unexplained muscle pain, tenderness, or weakness; fever, unusual tiredness; dark colored urine; chest pain; upper stomach pain, loss of appetite; or jaundice (yellowing of the skin or eyes). Common pravastatin side effects may include: muscle or joint pain; nausea, vomiting, diarrhea; headache; or cold symptoms such as stuffy nose, sneezing, sore throat.
SN instructed patient and caregiver regarding new medication Entresto. This medication is used to treat certain types of heart failure
. It may help you live longer and lower your chance of having to go to the hospital for heart failure
. This product contains 2 medications: sacubitril and valsartan. They work by relaxing blood vessels so that blood can flow more easily, which makes it easier for your heart to pump blood to your body. Cough, dizziness, or lightheadedness may occur. If any of these effects persist or worsen, tell your doctor or pharmacist promptly. To reduce the risk of dizziness and lightheadedness, get up slowly when rising from a sitting or lying position. If you miss a dose, take it as soon as you remember. If it is near the time of the next dose, skip the missed dose. Take your next dose at the regular time. Do not double the dose to catch up.
Patient has Dx of hypertensive heart disease without heart failure
. Hypertensive heart disease refers to heart conditions caused by high blood pressure. The heart working under increased pressure causes some different heart disorders. Hypertensive heart disease can cause thickening of the heart muscle, coronary artery disease, and other conditions such as heart failure
if not controlled. Follow your MD's orders for diet and medications closely
SN instructed patient about treatment of heart failure
may include: controlling risk factors: quitting smoking losing weight and increasing moderate exercise restricting salt and fat from the diet avoiding alcohol proper rest controlling blood sugar if diabetic controlling blood pressure limiting fluids.
SN did head to toe assessment and vital signs monitoring done. Received patient sitting on the chair with elevated blood pressure. SN instructed patient on the disease process, and management of CHF. Heart failure
occurs when the heart muscle doesn't pump blood as well as it should. When this happens, blood often backs up and fluid can build up in the lungs, causing shortness of breath. Certain heart conditions gradually leave the heart too weak or stiff to fill and pump blood properly. SN instructed patient to have a healthy lifestyle. Try to lose weight, exercise, use less salt and manage stress. SN will continue to assess and instruct the patient.
