Lymphedema Teaching 1987

SN instructed patient about Lymphedema disease. Lymphedema is the buildup of fluid in soft body tissues when the lymph system is damaged or blocked. When the lymph system is working as it should, lymph flows through the body and is returned to the bloodstream. Other conditions may cause the same symptoms. A doctor should be consulted if any of the following problems occur: Swelling of an arm or leg; which may include fingers and toes; a full or heavy feeling in an arm or leg; a tight feeling in the skin. Patient verbalized understanding the teaching.

Hypertension Teaching 2004

SN instructed patient and caregiver on hypertensive urgency which is a situation where the blood pressure is severely elevated and that experiencing hypertensive urgency may or may not experience one or more of these symptoms: severe headache, shortness of breath, nosebleeds, and severe anxiety, chest or back pain, numbness or severe weakness, change in vision or difficulty speaking. Patient and caregiver were advised to seek immediately medical assistance and/or call 9-1-1 if any of these signs or symptoms appear. Patient and caregiver verbalized understanding. Family is independent with hypertension process.

Liver failure Teaching 2018

SN instructed patient on liver failure. The early symptoms of liver failure are similar to symptoms of many other conditions. Because of this, liver failure may initially difficult to diagnose. Some of the most common initial symptoms of liver failure are: Nausea, Loss of appetite, fatigue, diarrhea. As liver failure progresses, the symptoms become more serious. The most common symptoms of advanced liver failure include: Skin itchiness, jaundice, bleeding easily, bruising easily, swollen abdomen, mental disorientation or confusion, sweet or foul smelling breath, sleepiness, coma. Educated patient that the liver filters the Ammonia out the blood and that liver failure causes Ammonia levels to increase, which cause change of mental status, confusion, and sleepiness or fatigue.

Torsemide Teaching 2025

SN instructed that Torsemide is a loop diuretic. It works by making the kidney eliminate larger amounts of electrolytes ( especially sodium and potassium salts ) and water than normal ( diuretic effect ). It is used to treat edema ( swelling ) associated with heart, kidney, or liver failure, or with conditions in which there is excess body water. It is also used alone or with other medicines to treat high blood pressure. Torsemide may increase the amount of urine or cause you to urinate more often when you first start taking it. To keep this from disturbing your sleep, try to take your dose before 6 pm. If you also take cholestyramine, do not take it within 4 hours before or after taking torsemide. may cause dizziness, lightheadedness, or fainting; alcohol, hot weather, exercise, or fever may increase these effects. To prevent them, sit up or stand slowly, especially in the morning. Sit or lie down at the first sign of any of these effects. Torsemide may affect your blood sugar. Check blood sugar levels closely. Weight should be monitored while taking this medication.

Restoril Teaching 2032

SN instructed patient about the Restoril ( temazepam ) is a benzodiazepine. Temazepam affects chemicals in the brain that may be unbalanced in people with sleep problems ( insomnia ). Restoril is used to treat insomnia symptoms, such as trouble falling asleep or staying asleep. Restoril works by slowing down the central nervous system ( brain ), causing drowsiness which helps patients fall asleep.

Ciprofloxacin HCL Teaching 2058

SN instructed patient regarding medication Ciprofloxacin which is the antibiotic prescribed by patient's physician for UTI. SN instructed patient that the medication's common side effects include nausea, vomiting, diarrhea and abdominal pain. SN Instructed patient to notify physician if any of these symptoms persist or worsen. Emphasized to patient the importance of completing the full course of the antibiotic regimen as prescribed to achieve the maximum therapeutic effect of the medication.

Amikacin Teaching 2078

SN instructed patient / caregiver on medication Amikacin. SN explained that Amikacin is an anti - infective in the aminoglycoside family which is indicated as part of combination therapy for mycobacterium avium complex. SN explained that side effects include ototoxicity, nephrotoxicity, apnea and hypersensitivity. SN instructed patient / caregiver that call your physician ( MD ) if you experience to signs of hypersensitivity, tinnitus ( ringing of the ears ), vertigo ( dizziness ), muscle weakness, twitching, feeling of fullness in the head or hearing loss.

Hydrochlorothiazide Teaching 2093

SN instructed patient / caregiver on medication Hydrochlorothiazide. SN explained to patient that Hydrochlorothiazide ( HCTZ ) is a medication indicated for edema and hypertension. SN explained to patient / caregiver that it is a diuretic that reduces fluid volume overload which, in turn, lowers blood pressure. Explained that side effects of this medication includes dizziness, dehydration, weakness, pancreatitis, anemia and nausea. SN instructed patient / caregiver to notify physician ( MD ) if experiencing persistence or worsening of side effects.

Denosumab Injection ( Xgeva ) Teaching 2118

SN instructed the use of Denosumab Injection ( Xgeva ) is used to treat osteoporosis a condition in which the bones become thin and weak and break easily in men and in women who have undergone menopause or who cannot take or did not respond to other medications for osteoporosis. Is also used to treat bone loss in men with prostate cancer and in women with breast cancer who are receiving certain treatments that increase their risk for fractures. In another cases is used to reduce fractures from certain types of cancer that began in another part of the body but has spread to the bones, is also used in adults and some adolescents to treat giant cell tumor of bone that cannot be treated with surgery or if surgery is not possible. Denosumab Injection ( Xgeva ) is in a class of medications called RANK ligand inhibitors. It works by decreasing bone breakdown and increasing bone strength and density .

Urinary Tract Infection Teaching 2141

SN instructed that the most commonly experienced symptom across individuals with UTIs is a burning sensation during urination. The passage through which urine passes to be expelled from the body becomes tender due to the infection; when urine goes through it, the person experiences a painful sensation that mostly doesn’t last post urination. Since in a lot of cases UTIs are not present with signs, if a burning pain is felt during urination, the infection may be strong, and thus seeking treatment at the earliest is recommended.
