Infection Teaching 1673

Patient educated that there is good and bad bacteria in our body’s and the ABT will kill both which result in the yeast infection. Yeast infection is identify by area is likely an area that is moist it is red and has raised pumps. Patient instructed on possible yeast infections related to long term use of ABT, in areas that include but is not limited to between legs, under breast, and in between groin. Patient instructed to report any symptoms to MD or SN during visit.

Acid reflux Teaching 1675

Instructed patient to avoid foods which might increase acid reflux. Food that is very hot or very cold, fried foods, peppermint, coffee, drinks that contain caffeine, spicy, highly seasoned foods, tomato-based dishes, citrus fruits and juices, chocolate and sweets. If patient any reflux symptoms, instructed on not lie down right after eating and to remain upright for at least 2 hours.

Hypertension Teaching 1676

Instructed patient and caregiver on Hypertensive urgency which is a situation where the blood pressure is severely elevated or higher for your diastolic pressure. That experiencing hypertensive urgency may or may not experience one or more of these symptoms: severe headache, shortness of breath, nosebleeds, and severe anxiety, chest pain, back pain, numbness/weakness, change in vision, difficulty speaking do not wait to see if your pressure comes down on its own. Seek immediately medical assistance and/or call 9-1-1.

PICC Line Teaching 1688

Skilled nurse remove PICC,line per doctor order, the catheter tip should also be examined and there is no breakage at the end, no S/S of infections noted. Skilled nurse applied at the insertion site with sterile gauze to prevent bleeding which and when the bleeding stops, the gauze is removed and a sterile dressing is applied. Instructed patient the dressing should remain for approximately 24 hours. After this time, the site should be assessed and a new dressing applied if needed. Patient understand the instructions given.

Carvedilol Teaching 1690

Carvedilol is used to treat heart failure (condition in which the heart cannot pump enough blood to all parts of the body) and high blood pressure. It also is used to treat people whose hearts cannot pump blood well as a result of a heart attack. Tell your doctor if any of these symptoms are severe or do not go away:tiredness, weakness, lightheadedness, dizziness, headache.

Foods Teaching 1722

SN instructed patient eat slowly in a relaxed atmosphere. Chew your food thoroughly. Avoid chewing gum, smoking, and drinking from a straw. This will help decrease the amount of air you swallow, which may help reduce gas.

SN advised caregiver to seek immediately medical assistance and/or call 9-1-1 if listed above occurs or worsen with patient. Caregiver verbalized understanding of all teachings during visit.
