Patient was instructed on strategies that can significantly help decrease the risk of a fall such as: Good lighting throughout the home, especially in stairwells and hallways, Non-slip floors and rugs, Hand rails on stairs, next to the toilet and in the shower and bathtub.
Instructed patient and caregiver about the procedures followed in the case of a hurricane. Should such event occur the Home Health Agency will be on call assisting any patient who has an emergency. Keep Agency phone number at hand.
Patient was instructed on hand-eye coordination. It
The patient was instructed in carpal tunnel release on care of the hands and dressings, to keep the arm and hand elevated above heart level to reduce swelling and healing. The patient was reviewed to check the dressing daily for signs of bleeding or drainage, when bathing are permitted the patient should be shower with the dressing covered by a plastic bag. The patient was recommended to follow prescribed exercises to maintain circulation and improve range of motion such as sponge squeezing.
The patient was instructed in carpal tunnel syndrome to avoid activities that increase stress on inflamed tissues, grasping and gripping action of hand and wrist, to keep the wrist in neutral position, to avoid the wrist in a bent flexed, twisted, turned position, and to minimize repetitive movements, including holding an object for extended periods of time. The patient was advised to wear a wrist splint to help keep the wrist in neutral position. The patient was taught on range-of-motion exercises and their importance for strengths of muscles.
Patient is unable to performed insulin injection by herself due to low vision and hand tremors. No caregiver able/willing to do that procedure due to ALF's policies. Nurse will continue search a c/g able and willing to administer insulin to patient.
Partient is unable for diabetic care due to multiples functional limitation such as poor vision, poor eyes/ hand coordination. No caregiver available at this time.
Instructed patient / caregiver strenuous work, with hand cannulated, lifting objects greater than ten pounds, or repetitive motion should all be avoided.
Patient also instructed on Clostridium difficile colitis (C. diff) and the increase risk due to the multiple ABT. Patient instructed that the most common symptoms of Clostridium difficile colitis (C. diff) infection include: watery diarrhea, fever, loss of appetite, nausea, belly pain and tenderness. Instructed to clean their hands with soap and water or an alcohol-based hand rub often and every after going to the restroom.
COPD Instructed patient about effective coughing by following these steps: While sitting in a chair, take several deep breaths as like for diaphragmatic breathing. Placing the hand over their stomach while breathing normally. Tightening up the stomach and chest muscles with the mouth open. Forcing air out while whispering the word "huff."
