Instructed caregiver reduce friction by making sure when lifting a patient in bed that they are
lifted, not dragged during repositioning, prevent ulcers from occurring and can also help them from
getting worse .
Make sure the skin remains clean and dry. Examine the skin daily. Inspect pressure areas gently. Make sure the bed linens remain dry and free of wrinkles. Pat the skin dry, do not rub
Partient is unable for diabetic care due to multiples functional limitation such as poor vision, poor eyes/ hand coordination. No caregiver available at this time.
Instructed patient caregiver knowledge of maintenance and care of PICC lines insures long term use. Problems, detected early, greatly decrease any complications that might be associated with this line.
Instructed patient about the signs and symptoms to call 9-1-1 with regard to diagnosis Congestive Heart Failure (CHF), or congestive heart failure, that may point to the condition being worsened and needing immediate medical attention by 9-1-1, call MD to report to MD: crackling noise that can be audibly heard as patient exhales, pink, frothy sputum that patient coughs out, patient has markedly increased shortness of breath that is not relieved even after 15 minutes of rest, may also be non-productive but incessant cough that includes crackling sound, rapid heart rate that may last more than 30 minutes (with or without dizziness), weight gain of more than 2-3 pounds in one day, or 5-7 pounds in seven days, lower extremity swelling which may or may not include fluid seeping through the skin. With regard to increased/increasing shortness of breath that is not relieved, however, told PCG and patient to remain calm and call 9-1-1 and not wait/call MD as it needs to be taken care of right away, because the patient needs oxygen in the body immediately - a medical emergency.
Instructed caregiver clean patient's skin daily: Clean the patient's skin around your tube 1 to 2 times each day.
Caregiver instructed about feeding and tube care to prevent a clogged feeding tube, flush your tube with water each time after giving a feeding or medication. If your feeding tube becomes clogged, you can use these methods: Place the syringe into your feeding tube, and pull back on the plunger. Flush your tube with warm tap water.
Skilled nurse teaching how patient performed colostomy care , washed the stoma itself and the skin around the stoma with soft paper towels, mild soap and water. Measured the stoma, cut out the opening, removed the paper back and set it aside. Finally hold the punch with the sticky side toward your body. Center the opening on the stoma , then press firmly abdomen for 30 seconds.
