That is used together with diet and exercise to treat Type 2 diabetes. Instructed patient to follow doctors orders and that it is to be taken 30 minutes before a meal twice a day. Reviewed symptoms / signs of hypoglycemia: headache, hunger, sweating, confusion, irritability, dizziness, or feeling shaky. Instructed on common side effects: mild nausea, diarrhea, constipation, dizziness, drowsiness or skin rash, redness or itching. Instructed patient to call 911 in the event of hives; difficulty breathing, swelling of your face, lips, tongue or throat.
SN instructed patient on ways to avoid acute exacerbations of chronic obstructive pulmonary disease ( COPD ) by recognizing early warning signs and then taking action to stop them in their tracks. The best way to do this is to work with your health care provider on an action plan so you know what to do to treat an exacerbation before it becomes serious. SN instructed on early warning signs of an acute exacerbation: Wheezing or more wheezing than what’s normal for you, coughing more than usual, shortness of breath that is worse than usual, an increase in the amount of mucus, change in the color of your mucus to yellow, green, tan, or bloody, shallow or rapid breathing, more than what’s normal for you, fever, confusion or excessive sleepiness, and swelling in your feet or ankles. SN instructed pt regarding Keppra (levetiracetam), it is an anti-epileptic drug, also called an anticonvulsant. Keppra is usually taken once every 12 to 24 hours. Take the medicine at the same time each day. You may take this medicine with or without food. Use Keppra regularly to get the most benefit. Get your prescription refilled before you run out of medicine completely. Do not stop using this medicine suddenly, even if you feel fine. Stopping suddenly may cause increased seizures. Follow your doctor's instructions about tapering your dose. Avoid drinking alcohol. It can increase some of the side effects of Keppra and may also increase the risk of seizures. Levetiracetam may impair your thinking or reactions. Avoid doing anything that requires your attention until you know how this medicine will affect you. Dizziness or severe drowsiness can cause falls or other accidents. Common Keppra side effects may include: dizziness, drowsiness; feeling tired; weakness; feeling aggressive or irritable; loss of appetite; stuffy nose; or infection. Call your doctor at once if you have: unusual changes in mood or behavior (unusual risk-taking behavior, being irritable or talkative); confusion, hallucinations, loss of balance or coordination; extreme drowsiness, feeling very weak or tired; bruising, severe tingling, numbness, pain, muscle weakness; problems with walking or movement; the first sign of any skin rash, no matter how mild; or signs of an infection - fever, swollen gums, painful mouth sores, pain when swallowing, skin sores, cold or flu symptoms, cough, trouble breathing. Pt verbalized understanding of uses and denies experiencing any SE. Pt stated he does not drink alcohol.

However, drinking more fluid than your body can process can reduce the amount of sodium (salt) in your blood. In the elderly, your body's fluid reserve becomes smaller, your ability to conserve water is reduced and your thirst sense becomes less acute. Symptoms are: extreme thirst, less frequent urination, dark-colored urine, fatigue, dizziness, confusion, patient verbalized understanding.
SN instructed patient about the drug lisinopril is a drug of the angiotensin-converting enzyme inhibitor class used primarily in treatment of hypertension ACE is important because it is an enzyme responsible for producing the chemical, angiotensin II. Angiotensin II causes muscles in most arteries, including the arteries of the heart, to contract, thereby narrowing the arteries and elevating blood pressure. ACE inhibitors such as lisinopril lower blood pressure by reducing the production of angiotensin II, thereby relaxing arterial muscle and enlarging arteries. When the blood pressure is lower, the heart - including the failing heart - does not have to work as hard to pump blood. The arteries supplying the heart with blood also enlarge during treatment with ACE inhibitors. This increases the flow of blood and oxygen to the heart, further improving the ability of the heart to pump bloodSide effects include: dizziness due to a drop in blood pressure, trouble breathing, blistering, peeling, red skin rash, confusion, weakness, uneven heartbeat, trouble breathing, numbness or tingling in your hands, feet, or lips, dark urine or pale stools. SN instructed patient/CG to seek medical attention if you have signs of an allergic reaction to Augmentin such as: hives; difficult breathing; swelling of your face, lips, tongue, or throat. Call your doctor at once if you have: severe stomach pain, diarrhea that is watery or bloody; pale or yellowed skin, dark colored urine, fever, confusion or weakness; loss of appetite, upper stomach pain, jaundice (yellowing of the skin or eyes); easy bruising or bleeding; little or no urination; or severe skin reaction - fever, sore throat, swelling in your face or tongue, burning in your eyes, skin pain followed by a red or purple skin rash that spreads (especially in the face or upper body) and causes blistering and peeling.

Common baclofen side effects may include: drowsiness, dizziness, weakness, tired feeling; headache; sleep problems (insomnia); nausea, constipation; or urinating more often than usual.
Instructed patient on Atorvastatin which is used to lower cholesterol, some side effects include confusion, memory problems; fever, unusual tiredness, and dark colored urine; swelling, weight gain, urinating less than usual or not at all, increased thirst, increased urination, hunger, dry mouth, and fruity breath odor.
