Lymphedema Teaching 1989

SN instructed patient and caregiver about Lymphedema disease. Lymphedema may be inherited ( primary ) or caused by injury to the lymphatic vessels ( secondary ). It is most frequently seen after lymph node dissection, surgery and/or radiation therapy, in which damage to the lymphatic system is caused during the treatment of cancer, most notably breast cancer. In many patients with cancer, this condition does not develop until months or even years, after therapy has concluded. Lymphedema may also be associated with accidents or certain diseases or problems that may inhibit the lymphatic system from functioning properly. In tropical areas of the world, a common cause of secondary lymphedema is filariasis, a parasitic infection. It can also be caused by a compromising of the lymphatic system resulting from cellulitis. Dementia Teaching 1996

SN instructed patient/ ALF that the dementia is not a specific disease. It's an overall term that describes a wide range of symptoms associated with a decline in memory or other thinking skills severe enough to reduce a person's ability to perform everyday activities.

Antibiotic Teaching 1999

SN instructed that it is tempting to stop taking an antibiotic as soon as you feel better. But the full treatment is necessary to kill the disease-causing bacteria. Failure to do so can result in the need to resume treatment later and may promote the spread of antibiotic - resistant properties among harmful bacteria. Patient verbalizes understanding.

Famotidine Teaching 2007

RN instructed patient about medication Famotidine that is used to treat gastroesophageal reflux disease, a condition in which backward flow of acid from the stomach causes heartburn and injury of the esophagus. RN instructed patient to swallow the tablets and capsules with a full glass of water. RN also explained about side effects of treatment such as: hives, skin rash, itching, swelling of the face, throat, tongue, lips, eyes, hands, feet, ankles, or lower legs. RN instructed to call Physician immediately if any of these signs or symptoms appear. Patient and caregiver voiced understanding of all instructions given.

Low fat diet Teaching 2008

RN instructed on low fat diet for treatment of high cholesterol and triglyceride levels. RN explained patient to avoid foods that are rich in fat / cholesterol, choose only lean meat and avoid the fat, eat more fish and poultry, have baked / broiled red meats, fish or poultry instead of fried, use low-fat or fat-free milk, try fat-free or low fat cottage cheese or yogurt in place of cream and sour cream, have steamed vegetables and dress salads with lemon juice, fat free mayonnaise or fat free dressing. RN also instructed patient that Atorvastatin reduces levels of "bad" cholesterol and triglycerides in the blood, while increasing levels of "good" cholesterol and is used to lower cholesterol and triglycerides in the blood and therefore it is used to lower the risk of stroke, heart attack and other heart complications in people with diabetes, coronary heart disease or other risk factors.

High blood pressure Teaching 2011

SN instructed patient on high blood pressure. The high blood pressure is a common condition and when not treated, can cause damage to the brain, heart, blood vessels, kidneys, and other parts of the body. Damage to these organs may cause heart disease, a heart attack, heart failure, stroke, kidney failure, loss of vision, and other problems. In addition to taking medication, making lifestyle changes will also help to control your blood pressure. These changes include eating a diet that is low in fat and salt, maintaining a healthy weight, exercising at least 30 minutes most days, not smoking, and using alcohol in moderation.

Stroke Teaching 2031

SN instructed patient / caregiver that the stroke occurs when a clot or a torn blood vessel in the brain stops blood from reaching a part of the brain. Damage to that part of the brain from lack of blood and oxygen can cause various signs and symptoms of stroke, such as facial drooping, numbness and paralysis. Although anyone can have a stroke, your risk increases if you're male, over age 65, or have one of these conditions: high blood pressure, high cholesterol, heart disease, or diabetes. Being overweight, smoking, abusing drugs or alcohol, and taking birth control pills increase risk, too. African - Americans, people who are Hispanic or Asian, and those with a close relative who's had a stroke are also at higher risk.

SN teaching patient Ventolin HFA ( albuterol sulfate inhalation aerosol ) is a bronchodilator used to treat or prevent bronchospasm in people with reversible obstructive airway disease. Ventolin HFA is also used to prevent exercise - induced bronchospasm.

SN instructed about Ventolin HFA ( albuterol sulfate inhalation aerosol ) is a bronchodilator used to treat or prevent bronchospasm in people with reversible obstructive airway disease. Ventolin HFA is also used to prevent exercise - induced bronchospasm. Ventolin HFA is an albuterol inhaler that helps relieve bronchospasm and its symptoms. Ventolin HFA acts within minutes, and symptom relief should last 4 to 6 hours. Albuterol treats bronchospasm by quickly opening the airways within your lungs to ease breathing during an asthma attack.

Spontaneous coronary artery dissection ( SCAD ) Teaching 2134

SN instructed patient on spontaneous coronary artery dissection ( SCAD ) and stent placement. A stent is a small rod made out of metal or fabric. It is inserted into a coronary artery when there is a blockage or when the artery is narrow. A coronary artery is a blood vessel that carries oxygen to the heart muscle. You received a stent due to coronary artery disease ( also called SCAD). That is caused by a thickening of the inside walls of the coronary arteries. This thickening is called atherosclerosis. A fatty substance called plaque builds up inside the thickened walls of the arteries, blocking or slowing the flow of blood. If your heart muscle doesn't get enough blood to work properly, you may have angina ( chest pain ) or a heart attack. The stent will help hold the artery open so blood can flow and help keep the artery from narrowing again. You will need to take medicine that keeps the stent open and blood flowing. It is very important to take the medicine exactly how your doctor tells you to take it. In addition to taking medicine, the best way to keep your heart healthy is to maintain a healthy lifestyle. Quit smoking. Make sure you keep your blood pressure and cholesterol at healthy levels. Exercise as recommended by your doctor. Eat a heart - healthy diet. Patient verbalized understanding well.
