Glucose meters Teaching 2016

SN instructed that the blood glucose monitors or glucose meters are devices individuals with diabetes use to monitor their blood sugar levels daily. Glucose meters give readings within five seconds of applying the test strip to a blood sample. For accurate readings, it is important to calibrate the blood glucose meter. Calibrating a meter is making sure the meter has the correct code to match the test strip. Some meters code automatically. Others, you must code or calibrate manually.

Stroke Teaching 2031

SN instructed patient / caregiver that the stroke occurs when a clot or a torn blood vessel in the brain stops blood from reaching a part of the brain. Damage to that part of the brain from lack of blood and oxygen can cause various signs and symptoms of stroke, such as facial drooping, numbness and paralysis. Although anyone can have a stroke, your risk increases if you're male, over age 65, or have one of these conditions: high blood pressure, high cholesterol, heart disease, or diabetes. Being overweight, smoking, abusing drugs or alcohol, and taking birth control pills increase risk, too. African - Americans, people who are Hispanic or Asian, and those with a close relative who's had a stroke are also at higher risk.

SN instructed patient / caregiver on medication Avandaryl. Explained that it is a medication indicated for Diabetes Mellitus ( DM ) type II to decrease the blood sugar by stimulating the production of insulin and increasing receptor site sensitivity. SN explained to patient / caregiver that the side effects include dizziness, photosensitivity, low blood sugar, constipation, nausea and vomiting. SN instructed patient / caregiver to monitor blood sugar daily and to not skip or double doses. SN instructed patient / caregiver to notify physician ( MD ) if experiencing unusual weight gain, swelling of ankles, drowsiness, dyspnea ( SOB ), muscle cramps, weakness, sore throat, rash, unusual bleeding or if bruising occurs.

Lantus insulin Teaching 2097

SN instructed patient / caregiver regarding medication Lantus insulin. SN instructed patient / caregiver that insulin is used by the body to remove and use glucose from the blood and produce energy and that it is used in type 2 diabetes to overcome the resistance of the cells to insulin. SN explained that Lantus insulin starts working after 1 - 2 hours and peaks after 6 hours after injection, and has a duration of 18 - 26 hours. SN informed that hypoglycemia is the most common side effect that may occur during insulin therapy. SN instructed patient / caregiver to rotate insulin injection sites to avoid bruising and injury.

Lipitor Teaching 2100

SN instructed patient regarding medication Lipitor. SN informed patient / caregiver that Lipitor is a medication that reduces levels of bad cholesterol ( LDL ) and triglyceride in the blood while increasing the level of good cholesterol ( HDL ). SN explained that this medication is used to treat high cholesterol and to lower risk of stroke, heart attack or other heart complication in people with type 2 diabetes. SN informed patient / caregiver that patient should eat low cholesterol diet and avoid drinking alcoholic beverages when taking lipitor because it increase risk of liver damage. SN explained to patient / caregiver that side effects to watch out for are unexplained muscle pain, fever and dark colored urine, swelling urinating less than usual and weight gain. SN instructed patient / caregiver to report any persistence or worsening of side effects.

SN explain that Heart failure is a condition in which the heart can not pump the amount of blood the body needs. Heart failure does not mean that the heart has stopped or is about to stop working. It indicates that the heart can not pump blood the way it should. It can affect one or both sides of the heart the weakening of the pumping capacity of the heart causes: Accumulation of blood and fluid in the lungs Accumulation of fluid in the feet, ankles and legs called edema Tiredness and shortness of breath The most common causes of heart failure are coronary artery disease, high blood pressure and diabetes. It is most common in people over 65, African - Americans, overweight people, and people who have already had a heart attack. Men are at greater risk than women.

SN explain about a couple of things going on that make your glucose rise in the morning. One of these is insulin resistance, a condition that means your body's muscle and fat cells are unable to use insulin effectively to lower blood glucose. However, insulin resistance also affects how your liver processes, stores, and releases sugar, particularly at night. The liver is supposed to release small amounts of glucose when you're not eating. But in type 2 diabetes, the liver dumps more glucose than is needed into the bloodstream, especially at night. So, while your hormones are causing a natural rise in blood glucose, your liver is releasing even more sugar into your system. Because your insulin resistance prevents your muscle and fat cells from using the sugar, your blood glucose level rises.

Sn instructed patient about diabetic wounds / ulcers have poor or delayed healing. Healing problems are caused by the peripheral arterial diseases and peripheral neuropathy that can occur with diabetes, wherein the small blood vessels in different parts of the body, especially in the extremities ( hands and feet ), grow narrower and reduce the blood circulation to those areas. A lack of circulation in the extremities can result in a reduced supply of oxygen and nutrients to the body tissue and nerves, which is necessary for healing. Over time, nerves in these areas may become damaged, decreasing the sensation of pain, temperature and touch, making patients vulnerable to injury.

Sn instructed about Jentadueto. This contains a combination of linagliptin and metformin. Linagliptin and metformin are oral diabetes medicines that help control blood sugar levels. Metformin works by decreasing glucose ( sugar ) production in the liver and decreasing absorption of glucose by the intestines. Linagliptin works by regulating the levels of insulin your body produces after eating.

Tresiba Teaching 2169

SN explained that the Tresiba is a long - acting insulin used to control high blood sugar in adults with diabetes. SN instructed about more frequents side effects of tresiba ( reactions at the injection site, itching, rash, allergic reactions , skin thickening or pits at the injection site ( lipodystrophy ), weight gain, and swelling of your hands and feet, among others). SN oriented to notify the doctor if any of these side effects are severe. SN instructed that the insulin dose may need to change because of change in level of physical activity or exercise, increased stress, change in diet, weight gain or loss, or illness.
