Medications
Instructed in new medication Plavix to reduce atherosclerotic events. In addition, warned of possible S/E such as headache, dizziness, fatigue, depression, pain, edema, hypertension, rhinitis, epistaxis, abdominal pain, gastritis, constipation, diarrhea, ulcers, arthralgia, coughing, rash, pruritus and others.
Instructed in new medication Vioxx to manage osteoarthritis. In addition, warned of possible S/E such as headache, asthenia, fatigue, dizziness, aseptic meningitis, hypertension, lower-extremity edema, sinusitis, diarrhea, dyspepsia, epigastric discomfort, heartburn, nausea, abdominal pain, GI bleeding, urinary tract infection, hyponatremia, back pain, bronchitis, upper respiratory tract infection, pulmonary edema and flu-like syndrome. Patient may experience signs and symptoms of GI bleeding, including bloody vomitus, blood in urine and stool and black, tarry stools. Advised to call MD if he experiences these signs or symptoms. Report rash, unexplained weight gain or edema. Avoid aspirin and products containing aspirin unless prescriber has instructed otherwise. Avoid OTC anti-inflammatories such as ibuprofen (Advil) unless prescriber has instructed it. All NSAIDs including Rofecoxib may adversely affect the liver. Signs and symptoms of liver toxicity include nausea, fatigue, lethargy, itching, jaundice, right upper quadrant tenderness and flu-like syndrome. Instructed to stop therapy and call MD immediately if patient experiences these signs or symptoms. Women must informMD if they become pregnant, or they are planning on becoming pregnant while taking drugs. This drug may be taken without regard to food although taking it with food may decrease GI distress.
The most common adverse effects of this drug are dyspepsia, epigastric discomfort, heartburn and nausea. Taking drug with food may help minimize these effects.
Instructed in new medication Zoloft to manage depression. In addition, warned of possible S/E such as headache, tremor, dizziness, insomnia, somnolence, paresthesia, hypoesthesia, fatigue, nervousness, anxiety, agitation, hypertonia, twitching, confusion, palpitations, chest pain, hot flashes, dry mouth, nausea, diarrhea, loose stools, dyspepsia, vomiting, constipation, thirst, flatulence, anorexia, abdominal pain, increased appetite, male sexual dysfunction, myalgia, rash, pruritus and diaphoresis.
Major side effects of Simvastatin include abdominal pain or cramps, blurred vision, dizziness, easy bruising or bleeding, itching, muscle pain or cramps, rash, and yellowing of the skin or eyes.
Isosorbide is used to prevent chest pain (angina) and reduce strain on the heart in patients with heart disease (coronary artery disease). Isosorbide mononitrate relaxes and widens blood vessels so blood can flow more easily. This medication will not relieve chest pain once it occurs. This medication may also be used in addition to other drugs to improve the symptoms of congestive heart failure (e.g., trouble breathing).

VITAMIN C
Patient was instructed in new medication Vitamin C side effects such as; nausea, vomiting, heartburn, abdominal cramps, and headache. Dental erosion may occur from chronically chewing vitamin C tablets.
SN instructed patient about Xarelto (rivaroxaban) is an anticoagulant (blood thinner) that prevents the formation of blood clots. Used to treat A-fib and lower the risk of a stroke from a blood clot. Side effects include bruising, itching, muscle pain, pain in arms or legs. Serious side effects include: urine that looks red, pink, or brown;headache, dizziness, weakness, feeling like you might pass out;bloody or tarry stools, coughing up blood or vomit that looks like coffee grounds.
SN instructed patient about Aggrenox contains a combination of aspirin and dipyridamole. Aspirin is in a group of drugs called salicylates. It works by reducing substances in the body that cause pain, fever, and inflammation. Dipyridamole keeps platelets in your blood from sticking together to form clots. Aggrenox is used to reduce the risk of stroke in people who have had blood clots or a "mini-stroke" (also called a transient ischemic attack or TIA).Avoid drinking alcohol while you are taking aspirin products. Alcohol may increase your risk of stomach bleeding. Side effects include joint pain, drowsiness, headache, upset stomach, mild heartburn, nausea, vomiting, or diarrhea.
Instructed patient and caregiver about Tramadol. It is an opioid pain medication used to treat moderate to moderately severe pain. Serious side effects of Tramadol may include: seizures, increased risk of serotonin syndrome, decreased alertness, and drug addiction. Common side effects include: constipation, itchiness and nausea, etc. A change in dosage may be recommended in those with kidney or liver problems.
SN instructed patient / caregiver about Ipratropium Bromide/Albuterol(Duoneb); get emergency medical help if you have any of these signs of an allergic reaction: hives; difficult breathing; swelling of your face, lips, tongue, or throat. Stop using albuterol and ipratropium and call your doctor at once if you have a serious side effect such as: wheezing, choking, or other breathing problems,chest pain, pounding heartbeats or fluttering in your chest, dangerously high blood pressure (severe headache, anxiety, uneven heart beat), swelling of your ankles or feet, eye pain, or seeing halos around lights, painful or difficult urination. Other common side effects may include: mild headache, or cold symptoms such as stuffy nose, sneezing, cough, or sore throat.
