Patient was instructed about Taztia ( Diltiazem ), it belongs to a group of drugs called calcium channel blockers. It works by relaxing the muscles of your heart and blood vessels. It is used to treat hypertension (high blood pressure), angina (chest pain), and certain heart rhythm disorders. Possible SE/AR may include, dizziness. When you sit or lie down for a while, get up slowly to allow your body to adjust and minimize dizziness. Also may also experience bloating, heartburn, muscle cramps, headache, flushing, nasal congestion, sore throat, constipation
or diarrhea. Inform your doctor if they become bothersome. Notify your doctor if you develop, breathing difficulties, swelling of the hands or feet, an irregular heartbeat.
SN instructed patient's CG about Quetiapine (Seroquel), it is an atypical antipsychotic approved for the treatment of schizophrenia, bipolar disorder, and along with an antidepressant to treat major depressive disorder. Possible SE/AR may include, dry mouth, dizziness, headache, somnolence. Some common adverse effects, high blood pressure, orthostatic hypotension, high pulse rate, high blood cholesterol, elevated serum triglycerides, abdominal pain, constipation
, increased appetite, vomiting, among others.
Instructed patient and caregiver about Tramadol. It is an opioid pain medication used to treat moderate to moderately severe pain. Serious side effects of Tramadol may include: seizures, increased risk of serotonin syndrome, decreased alertness, and drug addiction. Common side effects include: constipation
, itchiness and nausea, etc. A change in dosage may be recommended in those with kidney or liver problems.
SN instructed caregiver about Hydrocodone. Hydrocodone is used to treat moderate to severe pain and as an antitussive to treat cough. Common side effects of hydrocodone may include, nausea, vomiting, constipation
, drowsiness, dizziness, lightheadedness, fuzzy thinking, anxiety, abnormally happy or sad mood, dry throat, difficulty urinating, rash, itching, and narrowing of the pupils. Serious side effects include slowed or irregular breathing and chest tightness. Symptoms of hydrocodone overdose include narrowed or widened pupils, slow, shallow, or stopped breathing, slowed or stopped heartbeat, cold, clammy, or blue skin, excessive sleepiness, loss of consciousness, seizures, or death. Hydrocodone can be habit-forming, causing physical and psychological dependence.
SN instructed patient / caregiver that valacyclovir is an antiviral drug used in the management of herpes simplex, herpes zoster, and herpes B. Common adverse drug reactions are the same as for valacyclovir and include: nausea, vomiting, diarrhea and headache. Infrequent adverse effects may include: agitation, vertigo, confusion, dizziness, edema, arthralgia, sore throat, constipation
, abdominal pain, rash, weakness and/or renal impairment. Rare adverse effects include: coma, seizures, neutropenia, leukopenia, tremor, ataxia, encephalopathy, psychotic symptoms, crystalluria, anorexia, fatigue, hepatitis, Stevens–Johnson syndrome, toxic epidermal necrolysis and/or anaphylaxis.

SN instructed patient about Vicodin, it is indicated for relief of moderate to moderately severe pain of acute, chronic or postoperative types. Possible side effect / adverse reaction of narcotic analgesics are nausea, vomiting, constipation
, and dry mouth.
SN Instructed patient about the Ergocalciferol is vitamin D2. Vitamin D is important for the absorption of calcium from the stomach and for the functioning of calcium in the body.Possible hypervitaminosis D effects: polyuria, nocturia, polydipsia, hypercalciuria, reversible azotemia, hypertension, nephrocalcinosis, generalized vascular calcification, irreversible renal insufficiency (may be fatal), mental retardation, widespread soft tissue calcification, osteoporosis, dwarfism, vague aches, stiffness, weakness, nausea, anorexia, constipation
, mild acidosis, anemia, weight loss.
SN instructed patient / caregiver that the Tramadol is an opioid pain medication used to treat moderate to moderately severe pain. When taken as an immediate-release oral formulation, the onset of pain relief usually occurs within about an hour serious side effects may include seizures, increased risk of serotonin syndrome, decreased alertness, and drug addiction. A change in dosage may be recommended in those with kidney or liver problems. Its not recommended in women who are breastfeeding or those who are at risk of suicide. SN instructed patient / caregiver about Diphenhydramine( Benadryl). It is a first-generation antihistamine used to treat a number of conditions including allergic symptoms and itchiness, the common cold, insomnia, motion sickness, and extrapyramidal symptoms. It is a potent anticholinergic agent. This activity is responsible for multiples side effects such as: dry mouth and throat, increased heart rate, pupil dilation, urinary retention, constipation
, and, at high doses, hallucinations or delirium. Other side effects include motor impairment, flushed skin, blurred vision at near point owing to lack of accommodation, abnormal sensitivity to bright light (photophobia), sedation, difficulty concentrating, short-term memory loss, visual disturbances, irregular breathing, dizziness, irritability, itchy skin, confusion, increased body temperature, temporary erectile dysfunction, and excitability, and although it can be used to treat nausea, higher doses may cause vomiting. Report any Side effect / Adverse reaction to Physician / Nurse as needed.
