muscle weakness
Diseases Process
Caregiver A.L.F's staff was instructed in Hypothyroidism disease; on how symptoms differ among individuals, depending on the severity of the case: sensitivity to cold temperature, dry skin, constipation, forgetfulness, chronic fatigue, decreased heart rate, depression, hair loss, weight gain, muscle
stiffness and cramping, lack of facial expression, enlarged tongue.
SN instructed patient about the heart is a muscle
that pumps oxygen-rich blood to all parts of the body. When you have heart failure, the heart is not able to pump as well as it should. Blood and fluid may back up into the lungs (congestive heart failure), and some parts of the body don’t get enough oxygen-rich blood to work normally. These problems lead to the symptoms of heart failure. Follow a heart healthy diet. And make sure to limit the salt (sodium) in your diet. Salt causes your body to hold water. This makes your heart work harder as there is more fluid for the heart to pump.
SN also instructed pt on the disease process of hypothyroidism. The thyroid gland controls a lot of our body functions, when it is unable to produce the amount of thyroid hormone your body needs, you will need to take this hormone orally. Take this medication everyday as ordered by your MD. Report to SN or MD if you have sensitivity to cold temperature, dry skin, constipation, forgetfulness, chronic fatigue, decreased heart rate, depression, hair loss, weight gain, muscle
stiffness and cramping, as these can be signs your medication is not the correct dose.
SN instructed on arteriosclerosis is hardening and thickening of the walls of the arteries. This condition can occur because of fatty deposits on the inner lining of arteries (atherosclerosis), calcification of the wall of the arteries, or thickening of the muscular wall of the arteries from chronically elevated blood pressure. When arteriosclerosis affects the coronary arteries that supply blood to the heart muscle
, a shortage of oxygen delivered to the heart itself may lead to a heart attack.
Patient has Dx of hypertensive heart disease without heart failure. Hypertensive heart disease refers to heart conditions caused by high blood pressure. The heart working under increased pressure causes some different heart disorders. Hypertensive heart disease can cause thickening of the heart muscle
, coronary artery disease, and other conditions such as heart failure if not controlled. Follow your MD's orders for diet and medications closely
Instructed patient that Hypertensive heart disease refers to heart conditions caused by high blood pressure. The heart working under increased pressure causes some different heart disorders. Hypertensive heart disease can lead to complications such as : thickening of the heart muscle
, coronary artery disease, chronic kidney disease and other conditions such as heart failure if not controlled. Follow your MD's orders for Heart Healthy diet, exercise as tolerated and take medications as prescribed.
SN did head to toe assessment and vital signs monitoring done. Received patient sitting on the chair with elevated blood pressure. SN instructed patient on the disease process, and management of CHF. Heart failure occurs when the heart muscle
doesn't pump blood as well as it should. When this happens, blood often backs up and fluid can build up in the lungs, causing shortness of breath. Certain heart conditions gradually leave the heart too weak or stiff to fill and pump blood properly. SN instructed patient to have a healthy lifestyle. Try to lose weight, exercise, use less salt and manage stress. SN will continue to assess and instruct the patient.
