Urinary Tract Infection Teaching 1924

SN instructed patient / caregiver about urinary tract infection(uti), an infection in any part of the urinary system (kidneys, ureters , bladder and urethra). SN instructed patient / caregiver that most infections involve the lower urinary tract (the bladder and urethra). SN instructed symptoms / signs of uti such as a strong, persistent urge to urinate, a burning sensation when urinating, passing frequent, small amounts of urine. SN instructed patient / caregiver on possible causes such as infection of the urethra and bladder. SN instructed patient / caregiver on lifestyle/ home remedies such as drinking plenty of water daily to help flush the urinary tract, avoid holding it when there's need to go to prevent development of bacteria which can cause uti. SN instructed patient / caregiver to wipe from front to back after movement, to help prevent bacteria from the anus from entering the vagina or urethra.

Pain Teaching 1934

SN instructed patient to take pain medication before pain becomes severe to achieve better pain control. Always refill your meds before you run out of them. SN instructed patient on nonpharmacologic pain relief measures, including relaxation techniques, positioning ,etc. SN instructed to report to physician if experiencing pain level not acceptable , pain level greater than 6/10, pain medications not effective, unable to tolerate pain medications, and pain affecting ability to perform normal activities

Dementia Teaching 1936

SN instructed that the dementia symptoms vary depending on the cause, but common signs and symptoms include: Cognitive changes: Memory loss, difficulty communicating or finding words, difficulty with complex tasks, difficulty with planning and organizing, difficulty with coordination and motor functions, problems with disorientation, such as getting lost. Psychological changes like personality changes, inability to reason, inappropriate behavior, paranoia, agitation, hallucinations. Memantine (Namenda) works by regulating the activity of glutamate. Glutamate is another chemical messenger involved in brain functions, such as learning and memory. A common side effect of memantine is dizziness.

Skin Care Teaching 1966

SN instructed patient on importance of protecting skin by: keeping your skin moist with lotions or ointments to prevent cracking, wearing shoes that fit well and provide enough room for your feet, learning how to trim your nails to avoid harming the skin around them, wearing appropriate protective equipment when participating in work or sports.

Rom execises Teaching 1967

Why is it important to do active ROM exercises? ROM exercises are very important if you have been ill, injured, or must stay in bed or in a wheelchair. Active ROM exercises help build up or keep your muscles as strong as possible. They help keep your joints flexible ( bending and moving in the right way ). Doing ROM exercises will help keep good blood flow going to the joint area that is being exercised. They may help prevent blood clots. Exercising is natural and needed for the body to stay well. Your heart pumps stronger and easier when you exercise regularly. You also breathe quicker and more often while exercising. This helps prevent infections that happen if you do not breathe or cough deeply enough. With your heart and lungs working stronger, more blood gets to the muscles, bones, and skin. Increasing blood flow brings more oxygen and other good things to your body tissues and organs. Walking and other exercises help make your BMs and urine more regular. Exercising can also make you feel better, more relaxed, and happier.

Hypertension Teaching 1970

SN instructed patient about some measures aimed to managing & controlling hypertension, such as: eating low sodium diet , increase more fruits to increase your potassium, walk daily for 30 minutes, and have regular check-ups, as directed by Physician.

Carbohydrates Teaching 1971

SN instructed patient about pay attention to the carbohydrates because foods that contain carbohydrates raise blood glucose. Examples of foods that contain carbohydrates are pasta, bread, rice, desserts, juice, soda, and fruit.

Blood pressure Teaching 1978

SN instructed patient about some things that he can do to help control your blood pressure , including: Eat a heart-healthy diet, including potassium ( bananas ) and fiber ( cereals,beans ) , drink plenty of water, exercise regularly : walking for a few minutes every day , limit the amount of sodium ( salt ) you eat ( avoid canned foods ), reduce stress by trying to avoid things that cause you stress.

Rheumatoid arthritis Teaching 1992

Sn instructed patient while you may not be able to avoid the pain that comes with rheumatoid arthritis ( RA ), you can take action to limit it. By taking your pain medication on a schedule. Don’t wait until you are in more pain and have to play "catch-up." Using a warm, moist compress to loosen up a stiff joint. Try an ice pack on an inflamed joint. Massage may also help. These tried-and-true treatments are easy and can provide some quick relief for mild symptoms. Also instructed that on the importance to make it a priority every day to relax. If you need ideas for healthy ways to manage stress, ask your doctor or a counselor. You may want to try meditation, and focus on things you enjoy. Patient verbalizes understanding.

Hypertension Teaching 2004

SN instructed patient and caregiver on hypertensive urgency which is a situation where the blood pressure is severely elevated and that experiencing hypertensive urgency may or may not experience one or more of these symptoms: severe headache, shortness of breath, nosebleeds, and severe anxiety, chest or back pain, numbness or severe weakness, change in vision or difficulty speaking. Patient and caregiver were advised to seek immediately medical assistance and/or call 9-1-1 if any of these signs or symptoms appear. Patient and caregiver verbalized understanding. Family is independent with hypertension process.
