Wound Care
Patient was instructed on pain caused by pressure ulcers. Pain can
be classified as acute or chronic. Cyclic acute pain, which
is periodic and corresponds to the pain experienced during repeated management, such as dressing changes or patient repositioning and non-cyclic acute pain, which
is accidental, including
pain experienced during occasional procedures such as debridement or drain removal.
Patient was instructed on wounds contributing facts. In addition to poor circulation, neuropathy
, and difficulty moving, factors that contribute to chronic wounds include systemic illness, age and repeated trauma.
Instructed caregiver the patient are at high risk if the patient have or do the following: Neuropathy
, Poor circulation, A foot deformity (e.g., bunion, hammer toe), Wear inappropriate shoes, Uncontrolled blood sugar, History of a previous foot ulceration.
Instructed patient DO NOT massage the skin near or on the ulcer. This can
cause more damage
. DO NOT use donut-shaped or ring-shaped cushions. They reduce blood flow to the area, which
may cause sores.
Instructed in S/S of complications which
require need for medical intervention, including
redness, increase or change in drainage, heat at the wound site, fever, bleedind or increased pain.
Patient was instructed to help your ulcer heal more quickly, follow the advice below: try to keep active by walking regularly, whenever you're sitting or lying down, keep your affected leg elevated with your toes level with your eyes

Patient was instructed on traumatic wounds. Contusions are caused by more extensive tissue trauma after severe blunt or blast trauma. The overlying skin may seem to be intact but later become non-viable. Extensive contusion may lead
to infection.
