FOLEY CATHETER CARE
SN suggested patient to have nails, calluses and corns trimmed by podiatrist because those with diabetes have poor circulation. A good blood circulation is known to aid your body in all healing processes, so when it becomes sluggish, it takes longer for any injuries to on feet to heal.
SN instructed patient on s/s of infected wound susch as: Thick green or yellow drainage, Foul odor, Redness or warmth around wound, Tenderness of surrounding area, and Swelling.
SN instructed patient to eat a balanced diet and drink fluids, increase protein and take vitamins to promote wound healing.
SN advised patient to take temperature once a day before bedtime, check for bleeding, pus, hardness, swelling, odor and any color change. If any of these are present, please let your nurse or doctor know as soon as possible. Patient verbalized understanding of instructions given.
SN advised patient to call doctor immediately if any of the following happen: The cast gets wet, damaged, or breaks, skin or nails on the toes below the cast become discolored, such as blue or gray, skin, or toes below the cast are numb, tingling, or cold, the swelling is more than before the cast was put on, bleeding, drainage, or bad smells come from the cast, and severe or new pain occurs. Patient verbalized understanding of instructions given.
Instructed PCA the importance of the patient's skin care, keep the perineal area clean and change
pads and undergarments to prevent odors. Inspect the area regularly.
Skilled Nurse to educate on S/S of wound deterioration or infection such as: increase pain on wound site, swelling, temperature, and discharge.
Instructed the importance of adopting a lifestyle that controls an episode of Abdominal Aneurysm. Taught the patient how to care for the incision and instruct in case of any sign of infection, severe pain in the abdomen providing the name and telephone number of a physician or nurse.
Instructed patient how to change the dressings and how to care for the tubes and drains. Recommend to splint the abdomen when coughing or sneezing.
Advised the patient receives complete instructions in the care of the prosthesis. Instructed patient how to apply the elastic sleeve or wrap the stump for molding and shaping. Encouraged patient that the stump should not be wrapped too strongly, since this will prejudice circulation. Encouraged the importance of daily hygiene to prevent infection and skin breakdown.
